摘要
目的探讨Finkler超声评分联合血流阻力指数(resistance index,RI)在鉴别诊断良恶性卵巢肿瘤中的应用价值。方法收集2018年4月至2020年3月接受手术治疗卵巢肿瘤患者156例临床资料,回顾性分析二维及彩色多普勒超声检查结果。以术后病理检查结果为“金标准”,比较Finkler超声评分、RI、Finkler超声评分联合RI诊断效能。结果156例卵巢肿瘤患者中,恶性肿瘤62例(39.74%),良性肿瘤94例(60.26%)。卵巢肿瘤恶性组Finkler超声评分高于良性组(t=23.941,P<0.01),RI低于良性组(t=12.316,P<0.01)。Finkler评分法检出恶性卵巢肿瘤71例,良性卵巢肿瘤85例;RI检出恶性卵巢肿瘤67例,良性卵巢肿瘤89例;Finkler评分法联合RI检出恶性卵巢肿瘤65例,良性卵巢肿瘤91例。Finkler评分法联合RI诊断卵巢肿瘤特异度(92.55%)、准确度(92.95%)、阳性预测值(89.23%)高于Finler评分法80.85%、82.69%、74.65%(χ^(2)=5.852,7.671,4.810,P<0.05)。结论良恶性卵巢肿瘤Finkler超声评分、RI存在明显差异,Finkler超声评分联合RI能够提高卵巢良恶性肿瘤诊断效能。
Objective To investigate the application value of Finkler ultrasound score combined with blood flow resistance index(RI)in the differential diagnosis of benign and malignant ovarian tumors.Methods The clinical data of 156 patients with ovarian tumor who were treated by surgery in our hospital from April 2018 to March 2020 were retrospectively analyzed,including the examination results of two-dimensional and color Doppler ultrasonography.Taking the postoperative pathological examination results as the“gold standard”,the diagnostic efficacy of Finkler ultrasound score,RI and Finkler ultrasound score combined with RI was evaluated.Results Of the 156 patients,there were 62 cases(39.74%)of malignant tumors,94 cases of benign tumors.The Finkler ultrasound scores in malignant tumor group were significantly higher than those in benign tumor group(P<0.01),and RI was significantly lower than that in benign tumor group(P<0.01).Finkler ultrasound score detected 71 cases of malignant tumors,85 cases of benign tumors,and RI detected 67 cases of malignant tumors,89 cases of benign tumors,and Finkler ultrasound score combined with RI detected 65 cases of malignant tumors,91 cases of benign tumors.The specificity and accuracy as well as positive predictive value of Finkler ultrasound score combined with RI in diagnosis of ovarian tumor were 92.55%,92.95%,89.23%,respectively,which were significantly higher than those(80.85%,82.69%,74.65%)by Finkler score method(P<0.05).Conclusion There is significant differences in the differential diagnosis of benign and malignant ovarian tumors between Finkler ultrasound score and RI,and Finkler ultrasound score combined with RI can improve the diagnostic efficiency of benign and malignant ovarian tumors.
