肺炎衣原体感染与原发性肺癌的相关性

Association between chlamydia pneumoniae infection and primary lung cancer

摘要 目的 探究肺炎衣原体(Cpn)感染及血清Cpn抗体水平与原发性肺癌发病及临床特征关系。方法 选取2017年1月-2020年12月山东大学人民医院收治的120例原发性肺癌患者(肺癌组),匹配同期120名健康体检者(对照组),检测血清Cpn特异性抗体IgG、IgA阳性表达。探究血清Cpn-IgG、Cpn-IgA抗体水平与肺癌发生、血清肿瘤标志物[细胞角蛋白19片段(CTFRA21-1)、癌胚抗原(CEA)、神经元特异性烯醇化酶(NSE)、鳞状细胞癌抗原(SCCAg)]及临床特征关系。结果 肺癌组Cpn-IgG、Cpn-IgA、Cpn阳性率分别为60.83%、32.50%、62.50%,高于对照组的35.00%、8.33%、35.00%(P<0.05)。Cpn-IgG滴度>1∶512、1∶64~1∶512患者肺癌风险分别是Cpn-IgG滴度<1∶64的11.841倍、2.422倍;Cpn-IgA滴度≥1∶32患者肺癌风险是Cpn-IgA滴度<1∶32的4.533倍。肺癌Cpn阳性组患者血清CTFRA21-1、CEA、NSE、SCCAg表达均高于阴性组(P<0.05),肿瘤直径≥5 cm、临床分期Ⅲ~Ⅳ占比均高于阴性组(P<0.05)。结论 Cpn感染与原发性肺癌发病有关,随着Cpn-IgG、Cpn-IgA抗体滴度升高肺癌风险增加,且Cpn感染可能参与肺癌病情进展。 OBJECTIVE To explore the relationship between the Chlamydia pneumoniae(Cpn) infection,serum Cpn antibodies and the occurrence of primary lung cancer and clinical characteristics.METHODS Totally 120 patients with primary lung cancer who were treated in Jinan People′s Hospital from Jan 2017 to Dec 2020 were assigned as the lung cancer group,meanwhile,120 healthy people who received physical examination were chosen as the control group.The positive expressions of specific antibodies IgG and IgA of serum Cpn were detected.The association between the levels of serum Cpn-IgG,Cpn-IgA antibodies and the occurrence of lung cancer,serum tumor markers [cytokeratin 19 fragment(CTFRA21-1),carcinoembryonic antigen(CEA),neuron-specific enolase(NSE),squamous cell carcinoma antigen(SCCAg)] as well as clinical characteristics was observed.RESULTS The positive rates of Cpn-IgG,Cpn-IgA and Cpn of the lung cancer group were respectively 60.83%,32.50% and 62.50%,significantly higher than 35.00%,8.33% and 35.00% of the control group(P<0.05).The patients with Cpn-IgG titer more than 1∶512 was 11.841 times the risk for lung cancer as the patients with Cpn-IgG titer less than 1∶64,and the patients with Cpn-IgG titer ranging between 1∶64 and 1∶512 was 2.422 times the risk for lung cancer as the patients with Cpn-IgG titer less than 1∶64;the patients with Cpn-IgA titer no less than 1∶32 were 4.533 times the risk for lung cancer as the patients with Cpn-IgA titer less than 1∶32.The expression levels of serum CTFRA21-1,CEA,NSE and SCCAg of the Cpn-positive group were significantly higher than those of the Cpn-negative group(P<0.05).The proportions of patients with tumor diameter no less than 5 cm and patients with clinical stage ranging between Ⅲ and Ⅳ were significantly higher in the positive group than in the negative group(P<0.05).CONCLUSION The Cpn infection is associated with the occurrence of primary lung cancer,the risk for lung cancer is increased with the rise of Cpn-IgG,Cpn-IgA antibody titers,and the Cpn infection may involve in progression of lung cancer.
