摘要
目的:探讨经鼻间歇正压通气(NIPPV)与同步间歇指令通气(SIMV)在新生儿呼吸窘迫综合征(NRDS)治疗中的应用。方法:选取73例NRDS患儿为研究对象,按治疗方式不同分为NIPPV组(n=28)和SIMV组(n=45)。NIPPV组患儿进行NIPPV模式治疗,SIMV组进行SIMV模式治疗。观察比较两组临床疗效、临床相关指标、血气分析指标及并发症发生情况。结果:NIPPV组总有效率高于SIMV组(89.29%vs.68.89%,P<0.05);呼吸机使用时间、呼吸支持时间、住院时间均短于SIMV组(P<0.05)。治疗后12、24 h,两组患者PaO_(2)、PaCO_(2)、氧合指数均改善,且NIPPV组改善程度优于SIMV组(P<0.05)。NIPPV组并发症发生率低于SIMV组(10.71%vs.31.11%,P<0.05)。结论:NIPPV治疗NRDS具有较好疗效,可有效改善肺功能及缺氧症状,缩短治疗时间,且安全性较高,值得临床推广应用。
Objective:To explore the application of nasal intermittent positive pressure ventilation(NIPPV)and synchronous intermittent mandatory ventilation(SIMV)in the treatment of neonatal respiratory distress syndrome(NRDS).Methods:73 children with NRDS were selected as the study objects,and divided into NIPPV group(n=28)and SIMV group(n=45)according to different treatment methods.Children in NIPPV group were treated with NIPPV mode,and SIMV group were treated with SIMV mode.The clinical efficacy,clinical related indexes,blood gas analysis indexes and complications of the two groups were observed and compared.Results:The total effective rate in NIPPV group was higher than that in SIMV group(89.29%vs.68.89%,P<0.05).The ventilator use time,respiratory support time and hospital stay in NIPPV group were shorter than those in SIMV group(P<0.05).PaO_(2),PaCO_(2) and oxygenation index of the two groups were improved at 12 h and 24 h after treatment,and the improvement degree of NIPPV group was higher than that of SIMV group(P<0.05).The incidence of complications in NIPPV group was 10.71%,which was lower than 31.11%of SIMV group(P<0.05).Conclusion:NIPPV is effective in the treatment of NRDS,which can effectively improve pulmonary function and hypoxia symptoms,shorten the treatment time,and has high safety,which is worthy of clinical application.
