呼吸训练器联合床旁端坐法对ICU机械通气患者膈肌功能的影响 被引量:4

摘要 目的探究实施呼吸训练器联合床旁端坐法对ICU机械通气患者膈肌功能的影响。方法回顾性选取100例ICU患者为研究对象,根据随机法将研究对象分为对照组48例,观察组52例,对照组采取常规训练,观察组采取呼吸训练器联合床旁端坐法的护理模式,干预时间两周,两周后观察两组患者动脉血气指标改善情况、膈肌移动度、机械通气时间以及ICU住院时间、生活质量(SF-36)情况。结果①动脉血气指标:干预后观察组患者在动脉血CO_(2)分压(PaCO_(2))、肺泡血CO_(2)分压(PACO_(2))、动脉血O_(2)分压(PaO_(2))、肺泡血O_(2)分压(PAO_(2))好转程度优于对照组患者(P<0.05);②膈肌移动度:干预后观察组患者膈肌移动度较对照组提高(P<0.05);③机械通气时间以及ICU住院时间:干预后观察组机械通气时间以及ICU住院时间少于对照组患者(P<0.05);④SF-36:干预后观察组在机体疼痛、健康状况、活力、社会功能四方面评分高于对照组(P<0.05)。结论实施实施呼吸训练器联合床旁端坐法可改善ICU机械通气患者动脉血气指标、增加膈肌移动度、减少机械通气时间以及住院时间、提升生活质量,对于改进临床护理措施具有重要意义。 Objective To explore the effect of breath training device combined with bedside sitting on diaphragm function of ICU patients with mechanical ventilation.Methods A total of 100 cases of ICU patients were retrospectively selected as the research objects.According to the random method,the research objects were divided into control group(48 cases)and observation group(52 cases).The observation group received the nursing mode of breath training device combined with bedside sitting method,and the intervention time was two weeks.After two weeks,the improvement of arterial blood gas indexes,diaphragm mobility,mechanical ventilation time,ICU stay and quality of life(SF-36)were observed in both groups.Results①Arterial blood gas indexes:after intervention,the improvement degree of PaCO_(2),PACO_(2),PaO_(2) and PAO_(2) in observation group was better than that in control group(P<0.05).②Diaphragm mobility:after intervention,diaphragm mobility in the observation group was higher than that in the control group(P<0.05).③Duration of mechanical ventilation and ICU hospitalization:after intervention,the duration of mechanical ventilation and ICU hospitalization in the observation group were less than those in the control group(P<0.05).④SF-36:after intervention,the scores of pain,health status,vitality and social function in the observation group were higher than those in the control group(P<0.05).Conclusion The implementation of breath training device combined with bedside sitting method can improve the arterial blood gas indexes,increase diaphragm mobility,reduce the duration of mechanical ventilation and hospital stay,and improve the quality of life of ICU patients with mechanical ventilation,which is of great significance for improving clinical nursing measures.
