摘要
目的探讨可视喉镜下应用低温等离子切除会厌囊肿对会厌囊肿继发急性会厌炎患者病情发展和预后的影响。方法选择2015年7月~2020年6月于我院住院治疗的会厌囊肿继发急性会厌炎患者37例,其中23例患者在常规治疗基础上接受可视喉镜下低温等离子手术治疗,设为观察组;其余14例患者因拒绝手术给予常规治疗,设为对照组。比较2组患者的咽痛持续时间、住院时间及复发率,分析观察组囊液细菌培养结果。结果对照组患者咽痛持续(7.14±1.29)d,住院时间(8.79±1.42)d;观察组咽痛持续(6.21±0.95)d,住院时间(7.48±1.16)d,差异均有统计学意义(P值均<0.05)。随访期间,对照组急性会厌炎复发率为21.43%(3/14),观察组无复发,差异有统计学意义(P=0.047)。观察组囊液细菌培养阳性率为39.13%(9/23)。结论可视喉镜下应用低温等离子切除已受感染的会厌囊肿可促使会厌囊肿继发急性会厌炎患者病情的恢复,降低复发率,值得临床推广应用。
Objective To investigate the influence of resection of epiglottis cyst by video laryngoscopy combined with low-temperature plasma on the development and prognosis of acute epiglottitis secondary to epiglottis cyst.Methods Data of 37 patients with acute epiglottitis secondary to epiglottis cyst hospitalized in our hospital from July 2015 to June 2020 were analyzed.Among them,23 patients were treated with video laryngoscopy combined with low-temperature plasma(the observation group)and the other 14 patients refused surgery and accepted the routine treatment(the control group).The duration of angina,hospitalization time and recurrence rate of the two groups were compared.The bacterial culture results of the cyst fluid in the observation group were analyzed.Results There were significant differences in the duration of angina[(7.14±1.29)days in the control group and(6.21±0.95)days in the observation group]and hospitalization time[(8.79±1.42)days in the control group,(7.48±1.16)days in the observation group](P<0.05).During the follow-up period,the recurrence rate of acute epiglottitis in the control group was 21.43%(3/14),and no recurrence was recorded in the observation group(P=0.047).The positive rate of bacterial culture was 39.13%(9/23).Conclusions Low-temperature plasma resection of the infected epiglottis cysts under video laryngoscopy can promote the recovery of patients with epiglottic cysts sedondary to acute epiglottitis,and can reduce the recurrence rate,which is worthy of clinical application.
