摘要
目的探究内镜经黏膜下隧道肿瘤切除术(STER)治疗食管固有肌层肿瘤的效果及安全性。方法将我院收治的40例食管固有肌层肿瘤患者根据计算机随机化法分为对照组[20例,内镜黏膜下挖除术(ESE)治疗]和试验组(20例,STER治疗)。比较两组的治疗效果。结果试验组的手术出血量少于对照组,手术用时、住院时间短于对照组(P<0.05)。试验组的并发症总发生率低于对照组(P<0.05)。术后3 d,试验组的白细胞分化抗原4阳性(CD4^(+))、白细胞分化抗原4阳性/白细胞分化抗原8阳性(CD4^(+)/CD8^(+))高于对照组,白细胞分化抗原8阳性(CD8^(+))低于对照组(P<0.05)。术后3 d,试验组的白细胞介素-6(IL-6)、白细胞介素-1β(IL-1β)、肿瘤坏死因子-α(TNF-α)水平低于对照组(P<0.05)。结论STER治疗食管固有肌层肿瘤的效果显著,能有效优化手术指标,降低炎症因子水平及并发症发生率,提高患者的免疫功能,且安全性较好。
Objective To explore the efficacy and safety of submucosal tunneling endoscopic resection(STER)in the treatment of esophageal muscularis propria tumor.Methods According to the computer randomization method,40 patients with esophageal muscularis propria tumor admitted in our hospital were divided into control group[20 cases,endoscopic submucosal excavation(ESE)treatment]and experimental group(20 cases,STER treatment).The therapeutic effects of the two groups were compared.Results The amount of bleeding in the experimental group was less than that in the control group,and the operation time and hospitalization time were shorter than those in the control group(P<0.05).The total incidence of complications in the experimental group was lower than that in the control group(P<0.05).At 3 d after operation,the cluster of differentiation 4 positive(CD4^(+))and cluster of differentiation 4 positive/cluster of differentiation 8 positive(CD4^(+)/CD8^(+))in the experimental group were higher than those in the control group,and the cluster of differentiation 8 positive(CD8^(+))was lower than that in the control group(P<0.05).At 3 d after operation,the levels of interleukin-6(IL-6),interleukin-1β(IL-1β)and tumor necrosis factor-α(TNF-α)in the experimental group were lower than those in the control group(P<0.05).Conclusion STER in the treatment of patients with esophageal muscularis propria tumor has a significant effect,which can effectively optimize the surgical indicators,reduce the levels of inflammatory factors and the incidence of complications,improve the immune function of patients,and has good safety.
