摘要
目的:比较内镜黏膜下肿物挖除术(ESE)及内镜经黏膜下隧道肿瘤切除术(STER)治疗胃食管连接处固有肌层肿瘤的效果。方法:选取2021年10月—2024年3月烟台市福山区人民医院收治的胃食管连接处固有肌层肿瘤患者92例,采用随机数字表法分为研究组与对照组,每组46例。研究组应用STER,对照组行ESE。比较两组手术情况、术后住院时间及并发症发生情况。结果:两组病灶完整切除率、钛夹用量比较,无统计学差异(P>0.05);研究组病变切除操作时间、创面长径短于对照组(P<0.001),失血量少于对照组(P<0.001)。研究组术后住院时间短于对照组(P<0.001);两组并发症总发生率比较,无统计学差异(P>0.05)。结论:ESE及STER治疗胃食管连接处固有肌层肿瘤的效果及术后并发症发生率相当,但STER操作时间短,术中损伤小,术后住院时间短。
Objective:To compare the effect of endoscopic submucosal excavation(ESE)and submucosal tunneling endoscopic resection(STER)in the treatment of tumors originating from muscularis propria at the gastroesophageal junction.Methods:From October 2021 to March 2024,92 patients with tumors originating from muscularis propria at the gastroesophageal junction who were admitted to Fushan District People's Hospital of Yantai City were selected and divided into the study group and the control group according to the random number table method,with 46 cases in each group.The study group was treated with STER,and the control group was treated with ESE.The operation conditions,postoperative hospital stay and complications were compared between the two groups.Results:There were no statistical differences in the complete resection rate of lesions and the amount of titanium clips between the two groups(P>0.05).The operation time of lesion resection and the length of wound in the study group were shorter than those in the control group(P<0.001),and the blood loss in the study group was less than that in the control group(P<0.001).The postoperative hospital stay in the study group was shorter than that in the control group(P<0.001).There was no statistical difference in the total incidence of complications between the two groups(P>0.05).Conclusions:ESE and STER have similar efficacy and postoperative complication incidence in the treatment of tumors originating from muscularis propria at the gastroesophageal junction.However,STER has shorter operation time,less intraoperative injury and shorter postoperative hospital stay.
