摘要
目的探讨CT、MRI检查对盆腔恶性肿瘤的诊断价值。方法选取342例女性盆腔恶性肿瘤患者,均接受CT、MRI检查,以病理学检查结果为金标准,比较CT、MRI单独及联合检查对盆腔恶性肿瘤的诊断价值,包括影像学表现、病灶检出率、诊断符合率。结果病理学检查结果显示,342例盆腔恶性肿瘤患者中,卵巢癌162例,宫颈癌112例,子宫内膜癌68例;共检测出436个病灶,其中卵巢癌226个,宫颈癌134个,子宫内膜癌76个;肿瘤直径1.5~16.4 cm,平均(9.24±3.05)cm。MRI检查诊断卵巢癌、宫颈癌的符合率及病灶个数检出率均略高于CT检查,对子宫内膜癌的诊断符合率及病灶个数检出率均略低于CT检查,但CT检查与MRI检查诊断不同类型盆腔恶性肿瘤的符合率及病灶个数检出率比较,差异均无统计学意义(P﹥0.05);CT+MRI检查诊断盆腔恶性肿瘤的Kappa值、特异度、阳性预测值均高于二者单独检查。结论CT、MRI检查均能较好显示盆腔恶性肿瘤病灶的形态及与邻近组织的关系,均具有较高的病灶检出率和诊断符合率,临床可根据患者个体情况选择单独或联合检查。
Objective To explore the diagnostic value of CT and MRI in pelvic malignant tumor.Method A Total of 342 female patients with pelvic malignant tumor were selected and examined by CT and MRI.The value of single or combined CT and MRI in diagnosing pelvic malignant tumor including imaging manifestations,lesion detection rate and diagnostic coincidence rate were compared by using the pathological results as the gold standard.Result Pathological result showed there were 162 cases of ovarian cancer,112 cases of cervical cancer and 68 cases of endometrial cancer in342 patients with pelvic malignant tumor.A total of 436 lesions were detected in 342 patients with pelvic malignant tumor,including 226 ovarian cancer,134 cervical cancer,and 76 endometrial cancer;the tumor diameter was 1.5-16.4 cm,with an average of(9.24±3.05)cm.The coincidence rate of ovarian cancer and cervical cancer and detection rate of lesions by MRI were higher than those by CT,and endometrial cancer lower than that by CT,but there were no significant differences(P>0.05).The Kappa value,specificity and positive predictive value of CT and MRI combined detection were higher than those of single detection.Conclusion CT and MRI can show the shape of lesions and the relationship with adjacent tissues,and both have higher lesion detection rate and coincidence rate of pelvic malignant tumors,which can be used clinically alone or in combination according to the specific situation of patients.
