摘要
目的分析经阴道超声与剪切波弹性成像(SWE)技术对子宫内膜癌(EC)的诊断价值。方法选取98例EC患者作为EC组,另选取81例同期收治的子宫内膜良性疾病患者作为良性组,两组患者均进行经阴道超声及SWE检查。对比EC组与良性组患者的SWE参数值、血流分级情况;对比经阴道超声与SWE单独及联合检查诊断EC的效能。结果EC组患者杨氏模量平均值(E_(mean))、最大值(E_(max))、标准差(E_(sd))均明显高于良性组,差异均有统计学意义(P<0.01)。两组患者血流分级情况比较,差异有统计学意义(P<0.01),EC组患者血流分级以3级为主,良性组患者血流分级则以0级和1级为主。经阴道超声联合SWE与病理检查结果一致性极好(Kappa值=0.910);经阴道超声联合SWE诊断EC的灵敏度、特异度、阳性预测值、阴性预测值均高于经阴道超声及SWE单独检查。结论经阴道超声与SWE技术均可为EC诊断提供参考信息,而二者联合可提高对EC的诊断效能。
Objective To analyze the value of transvaginal ultrasound and shear wave elastic imaging(SWE)in the diagnosis of endometrial carcinoma(EC).Method A total of 98 EC patients were selected into the EC group,and 81 patients with benign endometrial diseases during the same period were selected into the benign group,the patients in both group were underwent transvaginal ultrasound and SWE examination.The SWE parameters,blood flow grading in EC group and benign group were compared.To compare the efficiency of transvaginal ultrasound and SWE alone and com-bined examination in diagnosing EC.Result The values of E_(mean),E_(max)and E_(sd)in the EC group were significantly higher than those in the benign group(P<0.01).There was significant difference for the blood flow grading between the two groups(P<0.01).The grade of blood flow in EC group was mainly grade 3,compared with grade 0-1 in the benign group.There was high consistency for transvaginal ultrasound combined SWE with pathological results(Kappa=0.910);the sen-sitivity,specificity,positive predictive value and negative predictive value of combined examination for EC diagnosis were higher than those of single examination.Conclusion Transvaginal ultrasound and SWE can provide reference for EC diagnosis,and the combined detection can improve the diagnostic efficiency of EC.
