摘要
目的探讨香砂六君子汤治疗脾胃虚弱型功能性消化不良的临床有效性及安全性。方法抽取2018年7月至2020年7月在我院就诊的80例脾胃虚弱型功能性消化不良患者进行临床研究,采用抽签法将其分为对照组和试验组,各40例。对照组单用多潘立酮片,试验组在对照组基础上加用香砂六君子汤。比较两组的临床疗效、临床症状缓解时间、中医证候积分、胃动力参数、脑肠肽指标、不良反应发生情况及生活质量评分。结果试验组的治疗总有效率为95.00%,明显高于对照组的77.50%,差异具有统计学意义(P<0.05)。试验组的上腹疼痛、食欲不振、早饱和上腹灼热缓解时间短于对照组,差异具有统计学意义(P<0.05)。治疗后,两组的脘腹痞满、食后腹胀、大便溏软、嗳气、纳差评分均低于治疗前,且试验组低于对照组(P<0.05)。治疗后,两组的胃动素(MTL)、餐前及餐后瘦素(LEP)水平及胃排空速率均高于治疗前,促肾上腺皮质激素释放激素(CRH)水平、2 h存留率低于治疗前,胃半排空时间短于对照组,且试验组优于对照组(P<0.05)。治疗后,两组的神经降压素(NT)、一氧化氮(NO)水平均低于治疗前,且试验组低于对照组(P<0.05)。试验组的不良反应总发生率低于对照组(P<0.05)。治疗后,两组的生活质量各项评分均高于治疗前,且试验组高于对照组(P<0.05)。结论香砂六君子汤治疗脾胃虚弱型功能性消化不良的效果显著,有助于缓解临床症状,改善胃动力参数、脑肠肽指标,降低不良反应发生率,提升生活质量。
Objective To explore the clinical efficacy and safety of Xiangsha Liujunzi decoction in the treatment of functional dyspepsia of spleen and stomach deficiency type.Methods Eighty patients with functional dyspepsia of spleen and stomach deficiency type treated in our hospital from July 2018 to July 2020 were selected for clinical study.The patients were divided into test group and control group by drawing lots,with 40 cases in each group.The control group was treated with domperidone alone,and the test group was treated with Xiangsha Liujunzi decoction on the basis of the control group.The clinical efficacy,clinical symptoms remission time,TCM syndrome scores,gastric motility parameters,brain gut peptide indexes,occurrence of adverse reactions and quality of life scores were compared between the two groups.Results The total effective rate of treatment in the test group was 95.00%,which was significantly higher than 77.50%in the control group,and the difference was statistically significant(P<0.05).The relief time of epigastric pain,loss of appetite,early satiety and epigastric burning in the test group were shorter than those in the control group,and the differences were statistically significant(P<0.05).After treatment,the scores of fullness of abdominal cavity,abdominal distension after eating,soft stool,belching and poor appetite in both groups were lower than those before treatment,and those of the test group were lower than the control group(P<0.05).After treatment,the levels of motilin(MTL),preprandial and postprandial leptin(LEP)and gastric emptying rate in both groups were higher than those before treatment,the levels of corticotropin-releasing hormone(CRH)and 2 h retention rate were lower than those before treatment,the gastric half-emptying time was shorter than that of the control group,and those of the test group were better than the control group(P<0.05).After treatment,the levels of neurotensin(NT)and nitric oxide(NO)in both groups were lower than those before treatment,and those of the test group were lower than the control group(P<0.05).The total incidence of adverse reactions in the test group was lower than that in the control group(P<0.05).After treatment,the each items scores of quality of life of the two groups were higher than those before treatment,and those of the test group were higher than the control group(P<0.05).Conclusion Xiangsha Liujunzi decoction in the treatment of functional dyspepsia of spleen and stomach deficiency type has a significant effect,which is helpful to relieve clinical symptoms,ameliorate gastric motility parameters and brain gut peptide indexes,reduce the incidence of adverse reactions,and improve the quality of life.
