摘要
目的对医联体基层医疗机构人员对专科医生参与家庭医生签约服务的认知分析,为进一步推动家庭医生签约服务提供参考。方法2021年5—6月,在皖北、皖中和皖南选取14个加入医联体的基层医疗机构进行问卷调查,调查基层医疗机构人员一般情况和对专科医生参与家庭医生签约服务认知情况。结果共调查有效样本量481人。69.4%基层医疗机构人员签约家庭医生服务;75.5%的调查对象所在团队有医联体核心医院专科医生参与;家庭医生签约服务的内容中,健康教育占比最高。基层医疗机构人员对医联体表示了解占57.6%,单因素分析显示性别、婚姻状况、聘用方式、是否签约家庭医生服务、地区5个因素对基层医疗机构人员了解医联体程度的影响差异有统计学意义(P<0.05)。94.8%的基层医疗机构人员赞同医联体核心医院专科医生参与家庭医生签约服务,地区变量对其影响差异有统计学意义(P<0.05)。结论基层医疗机构家庭医生签约情况较好,服务内容丰富且尚有提升空间;基层医疗机构人员赞同专科医生参与家庭医生签约服务,有利于深度合作;医联体内基层医疗机构与核心医院合作需求较大。建议多方面有针对性加强宣传与引导,增强对医联体政策的认可度;以需求为导向,完善与落实医联体内分级诊疗流程与制度。
Objective To analyze the cognition of the staff in primary health institutions of medical alliances on the participation of specialists in the family doctors’ contracted services, and to provide reference for further promoting the family doctors’ contracted services.Methods From May to June 2021, a questionnaire survey was conducted among the staff in 14 primary health institutions of medical alliances in northern, central and southern Anhui to investigate the general condition of the staff in these primary health institutions and their cognition of specialists’ participation in family doctors’ contracted services.Results There were a total of 481 valid responses. Totally 69.4% of the staff in primary health institutions signed for contracted family doctor services;75.5% of the respondents belonged to a team with the participation of doctors come from the core hospitals of medical alliances. Health education accounted for the highest proportion of contracted the services provided by family doctors. Altogether 57.6% of the staff in primary health institutions expressed their understanding of medical alliances. Univariate analysis showed that gender, marital status, employment mode, whether to sign a family-doctor service contract, and region had statistical differences in their influence on the staff’s understanding of medical alliances(P<0.05).And 94.8% of the staff in primary health institutions agreed that specialists in the core hospitals of medical alliances should participate in the contracted services of family doctors, and there was a statistical difference in the influence of regional variables(P<0.05).Conclusions The contract of family doctors in primary health institutions is good, the service content is rich and there exist room for improvement.The staff of primary health institutions agree that specialists’ participation in the contracted services of family doctors, which is conducive to in-depth cooperation.There is a great demand for the cooperation between primary health institutions and the core hospitals in medical alliances. It is suggested to strengthen the publicity and guidance in various aspects to enhance the recognition of the medical alliance policies, and to improve and implement hierarchical diagnosis and treatment procedures and systems within medical alliance on demand-oriented basis.
