摘要
目的 探讨贞芪扶正颗粒联合吉西他滨与顺铂化疗在非小细胞肺癌(NSCLC)合并肺结核(PTB)患者中的应用效果。方法 根据治疗方式的不同将80例NSCLC合并PTB患者分为对照组(n=32)和联合组(n=48),对照组患者给予吉西他滨与顺铂化疗,联合组患者在对照组的基础上给予贞芪扶正颗粒治疗。比较两组患者的总有效率、免疫炎性反应指标[CD4~+/CD8~+、辅助性T细胞(Th)1/Th2、C反应蛋白(CRP)、白细胞介素-6(IL-6)]、凝血功能指标[D-二聚体(D-D)、纤维蛋白原(FIB)]、肝肾功能指标[血清肌酐(Scr)、血尿素氮(BUN)、总胆红素(TBIL)、丙氨酸转氨酶(ALT)]以及治疗期间的不良反应发生情况。结果 治疗后,联合组患者的总有效率、CD4~+/CD8~+、Th1/Th2均高于对照组,CRP、IL-6、D-D、FIB、Scr、BUN、TBIL和ALT水平均低于对照组,差异均有统计学意义(P﹤0.05)。治疗期间,联合组患者的不良反应总发生率低于对照组(P﹤0.05)。结论 贞芪扶正颗粒联合吉西他滨与顺铂化疗可以提高NSCLC合并PTB患者的临床疗效,降低免疫炎性反应,改善凝血功能,减轻肝肾功能损伤,减少治疗期间的不良反应。
Objective To investigate the application effect of zhenqi fuzheng granules combined gemcitabine and cisplatin in patients with non-small cell lung cancer(NSCLC) complicated with pulmonary tuberculosis(PTB).Method A total of 80 patients with NSCLC+PTB were divided into control group(n=32,treated with gemcitabine^(+)cisplatin) and combined group(n=48,treated with zhenqi fuzheng granules + gemcitabine + cisplatin) according to different treatment methods.The total effective rate,immune inflammatory reaction indexes [CD4^(+)/CD8^(+),T helper cell(Th)1/Th2,C-reactive protein(CRP),interleukin-6(IL-6)],coagulation function indexes [D-Dimer(D-D) and fibrinogen(FIB)],liver and kidney function indexes [serum creatinine(Scr),blood urea nitrogen(BUN),total bilirubin(TBIL) and alanine aminotransferase(ALT)] and adverse reactions were compared between the two groups.Result After treatment,the total effective rate,CD4^(+)/CD8^(+)and Th1/Th2 in the combined group were higher than those in the control group,and the levels of CRP,IL-6,D-D,FIB,Scr,BUN,TBIL and ALT were significantly lower than those in the control group(P<0.05).During the treatment,the total incidence of adverse reactions in the combined group was significantly lower than that in the control group(P<0.05).Conclusion Zhenqi fuzheng granules combined gemcitabine and cisplatin can improve the clinical effect in NSCLC patients complicated with PTB,decrease immune inflammatory reaction,improve coagulation function,and reduce liver and kidney damage and adverse reactions during treatment.
