氨苄西林对孕晚期梅毒患者免疫功能和氧化应激及Th1/Th2因子水平的影响 被引量:5

Effect of ampicillin on immune function, oxidative stress and Th1/Th2 factor of third trimester patients with syphilis

摘要 目的探究氨苄西林对孕晚期梅毒患者免疫功能、氧化应激及辅助性T淋巴细胞1(Th1)/Th2因子水平的影响。方法选取2018年9月-2020年9月于西南医科大学附属中医院接受治疗的孕晚期梅毒患者107例,分为苄星青霉素组53例、氨苄西林组54例,分别使用苄星青霉素和氨苄西林连续治疗3周。比较两组患者皮损消退率、甲基胺红不加热血清试验(TRUST)转阴率及症状消退时间,检测T淋巴细胞亚群、氧化应激指标、Th1/Th2因子水平,分析不良妊娠结局发生率。结果氨苄西林组皮损消退率、TRUST转阴率高于苄星青霉素组(P<0.05),皮损消退时间较苄星青霉素组缩短(P<0.05)。治疗后,与苄星青霉素组比较,氨苄西林组CD_(8)^(+)、白细胞介素-2(IL-2)、IL-4、活性氧(ROS)、8-羟基脱氧鸟苷(8-OHdG)水平较低,CD_(4)^(+)、CD_(3)^(+)、γ干扰素(IFN-γ)水平较高(P<0.05)。与苄星青霉素组比较,氨苄西林组不良妊娠结局发生率较低,氨苄西林组新生儿1、5 min阿普加评分(Apgar)较高(P<0.05)。结论使用氨苄西林对孕晚期梅毒患者进行治疗,能够促进皮损等临床症状消退,减轻氧化应激损伤,调节Th1/Th2平衡,提升患者免疫功能,改善新生儿Apgar评分及妊娠结局,治疗效果显著。 OBJECTIVE To explore the effect of ampicillin on immune function,oxidative stress and helper T lymphocyte 1(Th1)/Th2 factor of the third trimester patients with syphilis.METHODS A total of 107 third trimester patients with syphilis who were treated in the Affiliated Traditional Chinese Medicine Hospital of Southwest Medical University from Sep 2018 to Sep 2020 were enrolled in the study and divided into the benzathine penicillin group with 53 cases and the ampicillin group with 54 cases,which were respectively treated with benzathine penicillin and ampicillin for 3 consecutive weeks.The regression rate of skin lesions,negative conversion ratio of methylamine red unheated serum test(TRUST)and remission time of symptoms were compared between the two groups of patients,the levels of T lymphocyte subsets,oxidative stress indexes and Th1/Th2 factor were detected,and the incidence rates of adverse pregnancy outcomes were observed.RESULTS The regression rate of skin lesions and negative conversion ratio of TRUST of the ampicillin group were significantly higher than those of the benzathine penicillin group(P<0.05),the remission time of skin lesions of the ampicillin group was shorter than that of the benzathine penicillin group(P<0.05).The levels of CD_(8)^(+),interleukin-2(IL-2),IL-4,reactive oxygen species(ROS)and 8-hydroxydeoxyguanosine(8-OHdG)of the ampicillin group were significantly lower than those of the benzathine penicillin group after the treatment,while the levels of CD_(4)^(+),CD_(3)^(+) and interferon-γ(IFN-γ)of the ampicillin group were significantly higher than those of the benzathine penicillin group(P<0.05).The incidence of adverse pregnancy outcomes of the ampicillin group was significantly lower than that of the benzathine penicillin group,and the neonatal 1 min and 5 min Apgar scores of the ampicillin group were significantly higher than those of the benzathine penicillin group(P<0.05).CONCLUSION Ampicillin may facilitate the remission of clinical symptoms like skin lesions,alleviate the oxidative stress injury,regulate the Th1/Th2 balance,boost the immune function,improve the neonatal Apgar score and pregnancy outcomes of the third trimester patients with syphilis,with the curative effect remarkable.
