摘要
动脉粥样硬化引起的斑块破裂、血栓形成是导致心血管病发病的病理基础,防治动脉粥样硬化是降低心血管病发病率、死亡率的关键。中医药通过调节免疫炎症反应防治动脉粥样硬化是目前研究的热点之一,以活血化瘀、化痰祛浊解毒等治法为主的复方、单体中药被证实可通过相关免疫炎症通路调控动脉粥样硬化的形成与发展,涉及的通路包括包括核因子κB通路、丝裂原活化蛋白激酶通路、Toll样受体通路、炎症小体通路等。炎症通路的复杂、交叉重合及中药药物成分的复杂性导致抗动脉粥样硬化的免疫炎症机制研究尚不明确,仍缺乏疗效评估的高级别循证医学证据。上述通路的研究仍然是目前的热点内容,尤其炎症小体通路值得未来深入探索,也为中药基于免疫抗炎治疗AS提供了新靶点和研究方向。
Atherosclerosis leads to plaque rupture and thrombus,which is the pathological basis of cardiovascular disease.Prevention and treatment of atherosclerosis are the key to reduce the morbidity and mortality of cardiovascular disease.Traditional Chinese medicine could prevent and treat atherosclerosis by regulating immune inflammatory response,which is one of the current research hotspots.Compound and monomer Chinese medicine,mainly used for promoting blood circulation and removing blood stasis,resolving phlegm and detoxifying,have been shown to regulate the formation and development of atherosclerosis through related immune inflammatory pathways,including NF-κB pathway,MAPKs pathway,TLR pathway and inflammasomes/caspases pathway.The complex and overlap of inflammatory pathways and the complex composition of traditional Chinese medicine lead to the unclear research on the immune inflammatory mechanism of anti-atherosclerosis,and there is still a lack of high-level evidence-based medical evidence for the efficacy evaluation of anti-atherosclerosis.The research of pathway is still a hot topic at present,especially the inflammasomes pathway,which providing new targets and research directions for traditional Chinese medicine based on immune anti-inflammatory treatment of atherosclerosis.
