重庆市某区基层公务员健康体检腰围身高比与血脂异常的相关性研究

摘要 目的探讨重庆市某区基层公务员人群腰围身高比(WHtR)与血脂异常的相关性,以及对血脂异常诊断的预测价值,以便更好地为基层公务员乃至范围更广的体检人群实施个性化健康管理。方法选取2020年9月至2021年1月在重庆市人民医院健康管理中心实施健康体检的重庆市某区基层公务员受检者1270例,以WHtR≥0.5为划界标准,将被研究的人群分成对照组(525例,WHtR<0.5)、肥胖组(745例,WHtR≥0.5),分析两组相关实验室检查血脂4项[总胆固醇(TC)、甘油三酯(TG)、低密度脂蛋白胆固醇(LDL-C)、高密度脂蛋白胆固醇(HDL-C)]的差异。比较两组受检者血脂异常的发生情况,以及WHtR与血脂异常的相关性。结果肥胖组受检者WHtR、TC、TG、LDL-C水平均高于对照组,HDL-C水平均低于对照组,差异均有统计学意义(P<0.05)。两组男性受检者血脂4项生化指标异常在总人数中占比高于女性,肥胖组受检者血脂异常发生率显著高于对照组,差异均有统计学意义(P<0.05)。结论WHtR≥0.5的受检者发生血脂异常的风险更高。因此,及时关注和监测WHtR,对预测血脂异常有重要意义,通过各种方式积极减轻体重,降低WHtR,有利于降低罹患高脂血症及心脑血管疾病风险。 Objective To explore Correlation between Waist-to-Height Ratio(WHtR)and dyslipidemia in health physical examination of grass roots civil servants in a district of Chongqing and its predictive value for the diagnosis of dyslipidemia,so as to better implement personalized health management for grass roots civil servants and even a wider range of physical examination groups.Methods A total of 1270 grassroots civil servants who conducted a physical examination in the Health Management Center of Chongqing People's Hospital from September 2020 to January 2021 were selected.Using WHtR≥0.5 as the demarcation standard,the studied population were divided into the control group(525 cases,WHtR<0.5)and the obesity group(745 cases,WHtR≥0.5).The differences of four items blood lipids[total cholesterol(TC),triglyceride(TG),low density lipoprotein cholesterol(LDL-C)and high density lipoprotein cholesterol(HDL-C)]between the two groups were analyzed.The incidence of dyslipidemia and the correlation between WHtR and dyslipidemia were compared between the two groups.Results The levels of WHtR,TC,TG,LDL-C in the obesity group were higher than those in the control group,and the level of HDL-C was lower than that in control group,and the differences were all statistically significant(P<0.05).The proportion of abnormal blood lipids in four biochemical indexes of the male subjects in the two groups were significantly higher than those of the female subjects,and the incidence of dyslipidemia in the obesity group was significantly higher than that in the control group,with statistical significance(P<0.05).Conclusion Subjects with WHtR≥0.5 have a higher risk of dyslipidemia.Therefore,timely attention to and monitoring of WHtR are of great significance for predicting dyslipidemia.Active weight loss in various ways and lower WHtR conducive to reducing the risk of suffering from hyperlipidemia and cardiovascular and cerebrovascular diseases.
