补阳还五汤对糖尿病肾病早期大鼠肾组织IL-12、IFN-γ等表达的作用研究 被引量:9

Effect of Buyang Huanwu Decoction(补阳还五汤)on the Expression of IL-12 and IFN-γin Renal Tissue of Diabetic Nephropathy Rats

摘要 目的观察补阳还五汤对糖尿病肾病大鼠肾组织的影响,探讨肾组织中白细胞介素-12(IL-12)、IFN-γ、白细胞介素-4(IL-4)、白细胞介素-10(IL-10)及ICAM-1、NF-kB表达变化与DKD关系,阐明补阳还五汤对糖尿病肾病的保护作用及可能机制。方法大鼠随机分组后,采用饲喂高糖高脂饲料联合腹腔注射链脲佐菌素制备糖尿病肾病早期大鼠模型;两中药组分别采用高、低剂量补阳还五汤(37.5 g·kg^(-1)、12.5 g·kg^(-1))灌胃4周,称体质量并查24 h尿蛋白定量、血清生化指标,于第14周末观察肾脏结构及炎细胞浸润程度,经免疫组化检测各样本肾组织中NF-kB、ICAM-1的表达变化,RT-PCR法检测各样本肾组织IL-12、IFN-γ、IL-4、IL-10mRNA表达变化。结果与空白组比较,造模后,糖肾早期模型组24 h尿蛋白尿,血清肌酐、尿素氮均有不同程度水平升高,肾组织中IL-4、IL-10基因表达水平降低,IL-12、IFN-γ基因表达上调(P<0.05或P<0.01)。药物干预4周后,和糖肾早期模型组相比,两中药组IL-4、IL-10基因表达略微上升,IL-12、IFN-γ基因表达水平降低,24 h尿蛋白尿较前下降(P<0.01),虽血清肌酐、尿素氮相比也略有降低,但差异无统计学意义(P>0.05);免疫组化提示NF-kB、ICAM-1在NC组中表达<中药低剂量干预组<中药高剂量干预组<糖尿病肾病早期模型组(P<0.01)。电镜病理提示给药组大鼠肾脏细胞形态结构明显改善。结论补阳还五汤对糖肾大鼠肾组织有保护作用,可能与下调大鼠肾组织中NF-kB、ICAM-1,IL-12、IFN-γ,上调IL-4、IL-10基因表达,减轻了免疫炎症反应对肾脏的损伤有关。 Objective To observe the effect of Buyang Huanwu Decoction(补阳还五汤)on renal tissue of diabetic nephropathy rats,and to explore the relationship between the expression of IL-12,IFN-γ,IL-4,IL-10,ICAM-1,NFkB with diabetic nephropathy,to elucidate the protective effect of Buyang Huanwu Decoction on DKD and its possible mechanism.Methods The SD rats were randomly divided into blank group,model group and experimental group;Diabetic nephropathy rat model was established by high sugar and high fat diet combined with intraperitoneal injection of streptozotocin(4Week);The high and low dose Buyang Huanwu Decoction(37.5 g·kg^(-1),12.5 g·kg^(-1))were given by gavage for 4 weeks.Then the body weight was weighed and 24-hour urine protein and serum biochemical indexes were examined,at the end of the 14th week,the renal structure and inflammatory cell infiltration were observed,the expression of ICAM-1 and NFkB in kiDKDey was detected by immunohistochemistry.The expression of IL-12,IFN-γ,IL-4 and IL-10 mRNA in kiDKDey was detected by RT-PCR.Results Compared with the normal group,the proteinuria,serum creatinine and urea nitrogen levels of the model group were increased in varying degrees,the gene expression levels of IL-4 and IL-10 in renal tissue were decreased,and the gene expression levels of IL-12 and IFN-γwere increased,after 4 weeks of administration,compared with the model group,the gene expression levels of IL-4 and IL-10 slightly increased,the gene expression levels of IL-12 and IFN-γdecreased,24-hour urine proteinuria decreased(P<0.01),serum creatinine and urea nitrogen slightly decreased,but the difference was not significant(P>0.05),immunohistochemistry showed that the expression of NFkB and ICAM-1 in NC group was lower than that in two traditional Chinese medicine intervention group(low-dose<high-dose)and early diabetic nephropathy model group(P<0.01).Electron microscopy showed that the morphology and structure of renal cells in the treatment group were significantly improved.Conclusion Buyang Huanwu decoction has protective effect on renal tissue of DKD rats,which may be related to down regulating NFkB,ICAM-1,IL-12,IFN-γ,up regulating the expression levels of IL-4,IL-10 and alleviating the renal injury caused by immune inflammatory reaction.
