摘要
目的:了解精神科急诊患者的一般人口学特征、病种分布及其诊疗情况。方法:回顾性调查2018年1月1日至2020年12月31日期间,在上海市精神卫生中心精神科急诊就诊患者的一般人口学资料和诊疗情况,对数据进行统计描述和分析。结果:3年内,精神科急诊患者就诊人次共计6616例,就诊患者数分别为1763例、2210例、2643例,呈逐年上升趋势。女性患者比例分别为917例(52%)、1253例(56.7%)与1542例(58.3%),差异具有统计学意义(P<0.05)。诊断主要为精神分裂症谱系、双相障碍谱系、兴奋躁动状态3类,分别占急诊就诊总人数的77.9%、78.9%及79.0%(P>0.05)。其中,双相障碍谱系占比有逐年上升趋势(18.5%、21.2%、25.2%,P<0.05)。在急诊用药方面,抗精神病药(78.0%、72.1%、72.8%)和苯二氮艹卓类药(36.9%、36.1%、37.8%)的使用频率较其他类药物高。结论:精神科急诊患者逐年增加,女性多于男性;以重性精神障碍为主。药物治疗有其特殊性,应加强对精神科急诊医学的重视和研究。
Objective:To explore the general demographic characteristics,disease distribution,diagnosis and treatment of psychiatric emergency patients.Methods:The patients in the psychiatric emergency department of Shanghai Mental Health Center from January 1st,2018 to December 31st,2020 were retrospectively investigated.The general demographic data,diagnosis and treatment of patients were collected.Descriptive statistics were used to analyze the data.Results:During the past three years,6616 patients visited the emergency department.The number of patients were 1763,2210 and 2643,with increasing gradually.The proportion of female patients were 917(52%),1253(56.7%)and 1542(58.3%),the differences were statistically significant(P<0.05).The main diagnosis was schizophrenia spectrum disorder,bipolar disorder and excited and restless state,which accounted for 77.9%,78.9%and 79.0%of the total number of emergency patients in the past three years(P>0.05).The proportion of bipolar disorder increased year by year(18.5%,21.2%,25.2%;P<0.05).In terms of emergency medication,the frequency of antipsychotics(78.0%,72.1%,72.8%)and benzodiazepines(36.9%,36.1%,37.8%)were higher than those of other kinds of drugs.Conclusion:The number of patients in psychiatric emergency department increases with years,with more female patients than men patients.The most common diseases are severe mental disorders.Drug treatment is so special that we should pay more attention to the psychosis and the emergency medicine treatment of psychiatric disease.
