摘要
目前冠心病仍是导致全球心血管患者病死的首要原因,且发病人群呈明显年轻化趋势,严重威胁了人类的身体健康。焦虑抑郁等情绪常伴发于冠心病患者,影响疾病的远期预后及患者的生活质量,由此西医提出“双心医学”模式关注患者的心理状态,但是西医应用传统的抗焦虑抑郁药物不良反应多,远期疗效差。而中医药在治疗冠心病合并焦虑抑郁方面已有着深厚的理论基础和丰富的临床实践经验,其注重“情志致病”,遵循“天人合一”理念,从阴阳五行学说、脏腑经络理论等方面阐述情志疾病的病因病机,主张治病当须“身心俱治”,通过辨证论治、经方古方、自拟方、中西医结合治疗及针灸、中医传统健身功法等诸多方法,对两病同时进行调整,具有依从性好、不良反应小、费用低、疗效良好等显著优势。通过概括近年来中医药干预冠心病合并焦虑或抑郁的病因病机、辨证论治要点和治疗概况,以期更好地为临床治疗冠心病合并焦虑抑郁提供参考。
At present,coronary heart disease is still the leading cause of death of cardiovascular patients in the world,and the incidence of coronary heart disease is obviously younger,which seriously threatens human health.Anxiety and depression often occur in patients with coronary heart disease,which affects the long-term prognosis of the disease and the quality of life of patients,so western medicine puts forward“psycho-cardiology”mode paying attention to the psychological state of patients,but traditional anti-depression drugs have many side effects and poor long-term effect.Traditional Chinese medicine has a profound theoretical basis and rich clinical experience in the treatment of coronary heart disease with anxiety or depression,it pays attention to“either unbearable”,and follows the idea of“nature and humanity”,This paper expounds the etiology and pathogenesis of emotional diseases from the theory of Yin and Yang,five elements theory,theory of Zang-fu organs and meridians,etc,advocates that the treatment of diseases should be based on the“physically”,and through syndrome differentiation and treatment,classical and ancient prescriptions,self-made prescriptions,treatment of integrated traditional Chinese and Western medicine,acupuncture,traditional Chinese medicine fitness methods etc,the two diseases were adjusted at the same time,which has the advantages of good compliance,small side effects,low cost and good curative effect.This paper will summarize the etiology and pathogenesis,key points of syndrome differentiation and treatment,and treatment overview of traditional Chinese medicine intervention in coronary heart disease with anxiety or depression in recent years,in order to better provide reference for clinical treatment of coronary heart disease with anxiety and depression.
