摘要
目的比较开放式与腹腔镜疝修补术治疗成人腹股沟疝的临床疗效。方法回顾性分析2018年7月至2020年7月辽阳市中心医院收治的成人腹股沟疝100例患者的临床资料,根据治疗方式的不同分为开放组(50例)和腔镜组(50例)。开放组(50例)患者接受开放式无张力疝修补术,腔镜组(50例)患者接受腹腔镜疝修补术。比较两组患者在手术疗效、术后恢复效果、患者的满意度以及术后并发症的总体发生率等情况。结果腔镜组的住院时间(6.70±1.10)d、手术时间(84.60±9.80)h、下床时间(1.30±0.40)d、术后疼痛持续时间(20.20±2.00)h、术中失血量(16.60±4.80)mL均少于开放组(P<0.05);腔镜组的治疗效果满意度98%高于开放组(P<0.05);腔镜组的术后并发症总体发生率4%低于开放组(P<0.05)。结论治疗成人的腹股沟疝,腹腔镜疝修补术(TAPP)较传统开刀无张力修补术的优势明显,不仅术后疼痛时间,住院时间、手术时间短,离床活动早,术中失血量少,术后康复快,而且可以提高患者的满意度,降低术后并发症的总体发生率,具有显著的临床应用价值,值得在临床中推广。
Objective To compare the clinical effect of laparoscopic herniorrhaphy and open-type herniorrhaphy in the treatment of adult inguinal hernia.Methods The clinical data of 100 patients with adult inguinal hernia admitted to Liaoyang Central Hospital from July 2018 to July 2020 were retrospectively analyzed,and they were divided into open group(50 cases)and laparoscopic group(50 cases)according to the different surgical methods.Laparoscopic herniorrhaphy was used in the laparoscopic group,and open-type tension-free herniorrhaphy was used in the open group.The two groups were compared in surgical efficacy,postoperative recovery effect,patient satisfaction and the overall incidence of postoperative complications.Results The length of hospital stay(6.70±1.10)days,operation time(84.60±9.80)hours,out of bed time(1.30±0.40)days,postoperative pain duration(20.20±2.00)hours,intraoperative blood loss(16.60±4.80)mL were all shorter in the laparoscopic group than in the open group(P<0.05);The satisfaction of treatment effect in the laparoscopic group was 98% higher than that in the open group(P<0.05),and the total incidence of postoperative complications in the laparoscopic group was 4% lower than that in the open group(P<0.05).Conclusion The treatment of adult inguinal hernia,laparoscopic hernia repair(TAPP)has obvious advantages over traditional tension-free repair,it can short postoperative pain time,short hospital stay,short operation time,early movement out of bed,less intraoperative blood loss and fast postoperative recovery,improve patient satisfaction and reduce the overall incidence of postoperative complications.It has significant clinical application value and is worth popularizing in clinical practice.
