摘要
目的观察电针联合益肾化痰汤治疗肾虚痰湿型多囊卵巢综合征(PCOS)的临床疗效及其对卵巢超声参数、内分泌激素及血清抗苗勒氏管激素(AMH)水平的影响。方法选取肾虚痰湿型PCOS患者86例,采用随机数字表法随机分组为对照组和研究组,每组43例。对照组予口服益肾化痰汤治疗,研究组在对照组口服中药基础上联合电针治疗。比较两组临床疗效和治疗前后卵巢超声参数、血清性激素[睾酮(T)、雌二醇(E2)、黄体生成激素(LH)和卵泡刺激素(FSH)]以及血清生殖激素[AMH、抑制素B(IHNB)]的水平变化。结果研究组临床疗效优于对照组(Z=4.440,P=0.035)。治疗后,两组卵巢超声参数髓质面积(SA)、卵巢体积、卵巢总面积(TA)、SA/TA均较同组治疗前降低(P<0.05),且研究组上述各项均明显低于对照组(P<0.05)。治疗后,两组血清T、FSH均较同组治疗前降低(P<0.05),且研究组上述两项明显低于对照组(P<0.05)。治疗后,两组血清E2、LH均较同组治疗前升高(P<0.05),且研究组上述两项明显高于对照组(P<0.05)。治疗后,两组血清AMH、IHNB均较同组治疗前降低(P<0.05),且研究组上述两项明显低于对照组(P<0.05)。结论电针联合益肾化痰汤治疗PCOS,可有效改善卵巢超声参数、血清性激素和生殖激素水平,改善排卵功能,提高临床疗效。
Objective To observe the clinical efficacy of electroacupuncture plus kidney-reinforcing and phlegmresolving decoction for polycystic ovarian syndrome(PCOS)of kidney-deficiency and phlegm-dampness type and its effect on ovarian ultrasound parameters,endocrine hormones and serum anti-Müllerian hormone(AMH)levels.Method Eighty-six patients with PCOS of kidney-deficiency and phlegm-dampness type were allocated,using a random number table,to a control group and a study group,with 43 cases in each group.The control group received oral administration of kidney-reinforcing and phlegm-resolving decoction and the study group received electroacupuncture in addition.The clinical therapeutic effects were compared between the two groups.Ovarian ultrasound parameters,serum sex hormone[testosterone(T),estradiol(E2),luteinizing hormone(LH)and follicle-stimulating hormone(FSH)]levels and reproductive hormone[AMH and inhibin B(IHNB)]levels were compared between the two groups before and after treatment.Result The total efficacy rate was higher in the study group than in the control group(Z=4.440,P=0.035).After treatment,ovarian ultrasound parameters medullary substance area(SA),ovarian volume,total ovarian area(TA)and SA/TA decreased in the two groups compared with before(P<0.05)and were significantly lower in the study group than in the control group(P<0.05).After treatment,serum T and FSH levels decreased in the two groups compared with before(P<0.05)and were significantly lower in the study group than in the control group(P<0.05).After treatment,serum E2 and LH levels increased in the two groups compared with before(P<0.05)and were significantly higher in the study group than in the control group(P<0.05).After treatment,serum AMH and IHNB levels decreased in the two groups compared with before(P<0.05)and were significantly lower in the study group than in the control group(P<0.05).Conclusion Electroacupuncture plus kidney-reinforcing and phlegm-resolving decoction can effectively improve ovarian ultrasound parameters,serum sex hormone and reproductive hormone levels,ovulatory function and clinical therapeutic effect in treating polycystic ovarian syndrome.
