摘要
目的探讨急诊急救过程中一种快速简单的治疗部分甲床缺损的新方法及疗效。方法2016年12月-2020年6月,对91例(106指/趾)部分甲床缺损病患(伤后6~8 h就诊,甲床缺损<5 mm)进行治疗。其中医用胶组46例(56指/趾),于急诊清创后使用医用胶(主要成分α-氰基丙烯酸正丁酯)对缺损创面进行一期封闭治疗,纱布覆盖疏松包扎;对照组45例(50指/趾),急诊清创后使用油纱布覆盖加压包扎行常规伤口换药治疗。结果对全部患者进行3~6个月的临床观察,医用胶组指甲愈合优良率为96.4%,对照组为78.0%,医用胶组指甲愈合效果优于对照组,差异有统计学意义(P<0.05)。结论医用胶可以促进缺损甲床的再生和修复,可以快速彻底止血缺损创面的同时一期封闭创面,降低伤口感染和疼痛,该方法是一种快速简单有效经济的甲床缺损修复方法。
Objective To explore a fast and simple new method for the treatment of partial nail bed defect in the process of emergency treatment and its curative effect.Methods From December 2016 to June 2020,91 cases with partial nail bed defect of 106 fingers/toes(coming to the hospital within 6-8 hours after injury,nail bed defect<5 mm)were enrolled.They were divided into medical glue group(56 fingers/toes of 46 patients)and control group(50 fingers/toes of 46 patients).In the medical glue group,after emergency debridement,the defects were treated with medical glue(main component:α-cyanoacrylate)for one-stage closure,covered with gauze and loosely bandaged.After emergency debridement,the patients in the control group were covered with oil gauze and bandaged with pressure.Results Clinical observation was carried out for 3-6 months,the excellent and good rate of nail healing was 96.4%in the medical glue group and 78.0%in the control group.The nail healing effect of the medical glue group was better than that of the control group,and the difference was statistically significant(P<0.05).Conclusion Medical glue can promote the regeneration and repair of nail bed,quickly and completely stop bleeding,and at the same time,seal the wound to prevent bacterial infection and reduce the wound pain.This method is a fast,simple,effective and economical method for repairing nail bed defect.
