摘要
经自然腔道内镜手术(natural orifice transluminal endoscopic surgery,NOTES)被认为是腹腔镜手术之后最重要的、发展潜力巨大的微创手术技术。近年来NOTES在多个手术领域崭露头角,并且不断发展创新,衍生出许多新的技术手段。有关术中面临到的感染、切口封闭等问题也得到了进一步的改善和保障,使大量的消化道疾病患者从中受益。目前暂无关于NOTES手术创面愈合方面的系统综述,本文主要以经肠NOTES肠道创面愈合的影响因素为出发点,围绕该手术的各种操作,如术前精细化肠道准备,术中腔内结肠闭塞装置的使用,肠壁切口的多种闭合方式等方面对创面愈合的潜在影响进行探讨,旨在为NOTES的更安全、高效发展提供一种新的视角。
Natural orifice transluminal endoscopic surgery(NOTES)is considered to be the most important minimally invasive surgical technique with great development potential after laparoscopic surgery.In recent years,NOTES has emerged in many surgical fields,and has continuously developed and innovated,deriving many new technical means.The problems of infection and incision closure during surgery have also been further improved and guaranteed,which brings lots of benefits to many patients with digestive tract diseases.There is no systematic review on wound healing of NOTES surgery.This article discussed the potential effects of various operations on wound healing,such as preoperative fine intestinal preparation,the use of intraluminal colon occlusion device during operation,and multiple closure methods of intestinal wall incision,from the direction of influencing factors of wound healing of intestinal incision after the trans-rectal pure NOTES,in order to provide a new angle for the safer and more efficient development of NOTES.
