授权赋能理论指导下健康教育对有心血管疾病症状的围绝经期女性应对方式 心理韧性及睡眠质量的影响 被引量:38

The intervention effect of health education on the coping style,mental toughness and sleep quality of perimenopausal women with cardiovascular disease symptoms guided by the empowerment theory

摘要 目的探讨授权赋能理论指导下健康教育对有心血管疾病症状的围绝经期女性应对方式、心理韧性及睡眠质量的影响。方法选取2017年12月—2019年12月该院心血管内科80例围绝经期女性,按照建档顺序分组,各40例。对照组采取常规健康教育,观察组基于对照组采取授权赋能理论下健康教育,连续干预3个月。统计两组健康知识掌握率、护理满意度,对比干预前后应对方式[特质应对方式问卷(TSCQ)]、心理韧性[心理韧性量表(CD-RISC)]、自护能力[自护能力量表(ESCA)]、睡眠质量[匹兹堡睡眠质量量表(PSQI)]。结果健康知识掌握率:观察组健康知识掌握率高于对照组(92.50%vs.70.00%,P<0.05);CD-RISC评分:干预1个月后、3个月后观察组自强、坚韧及乐观评分高于对照组(P<0.05);TSCQ评分:干预1个月后、3个月后观察组积极应对评分高于对照组,消极应对评分低于对照组(P<0.05);ESCA评分:干预1个月后、3个月后观察组自我护理技能、自我概念及自护责任感评分高于对照组(P<0.05);PSQI评分:干预1个月后、3个月后观察组日间功能、睡眠质量及入睡时间评分低于对照组(P<0.05);护理满意度:观察组护理满意度高于对照组(97.50%vs.75.00%,P<0.05)。结论授权赋能理论指导下健康教育应用于就诊于心血管内科的围绝经期女性,可提高健康知识掌握率及心理韧性,建立积极应对方式,增强自护能力,改善睡眠质量,减少头晕、胸闷等不适主诉,提升护理满意度。 Objective To explore the impact of health education on the coping style,mental toughness and sleep quality of perimenopausal women with cardiovascular disease symptoms under the guidance of empowerment theory.Methods From December 2017 to December 2019,80 cases of perimenopausal women in the Department of Cardiovascular Medicine in our hospital were selected and grouped according to the order of filing,with 40 cases in each group.The control group took regular health education,and the observation group based on the control group took health education under the theory of empowerment and intervened for 3 months.The mastery rate of health knowledge and nursing satisfaction degree were counted in the two groups,and the coping styles before and after the intervention [Trait Coping Style Questionnaire(TSCQ)],mental resilience [Psychological Resilience Scale(CD-RISC)],self-care ability [Exercise of Self-Care Agency Scale(ESCA)],and sleep quality [Pittsburgh Sleep Quality Scale(PSQI)] were compared.Results The mastery rate of health knowledge:The mastery rate of health knowledge in the observation group washigher than that in the control group(92.50% vs.70.00%,P<0.05).CD-RISC score:The self-improvement,tenacity,and optimism scores of the observation group after 1 month and 3 months of intervention were higher than those of the control group(P<0.05).TSCQ score:The positive response score of the observation group after 1 month and 3 months of intervention was higher than that of the control group,and the negative response score was lower than that of the control group(P<0.05).ESCA score:The self-care skills,self-concept,and self-care responsibility scores of the observation group were higher than those of the control group after 1 month and 3 months after the intervention(P<0.05).PSQI score:The scores of daytime function,sleep quality,and time to fall asleep in the observation group after 1 month of intervention and 3 months later were lower than those of the control group(P<0.05).The degree of nursing satisfaction:97.50% of the nursing satisfaction of the observation group was higher than that of the control group(97.50% vs.75.00%,P<0.05).Conclusion The application of health education under the guidance of the empowerment theory to perimenopausal women in Department of Cardiovascular Medicine can improve the grasping rate of health knowledge and mental toughness,establish active coping styles,enhance self-care ability,improve sleep quality,reduce the complaints of dizziness,chest tightness and other discomforts,and improve the degree of nursing satisfaction.
