摘要
目的观察重症急性胰腺炎(SAP)应用早期肠内营养(EN)的治疗效果。方法选取2019年4月至2021年4月联勤保障部队第九〇〇医院莆田医疗区收治的60例SAP患者,按照随机数字表法分为对照组(n=30)与干预组(n=30),对照组接受延迟肠内营养,干预组接受早期肠内营养,比较两组的临床指标、实验室指标、健康评分、感染率、并发症发生率及病死率。结果干预组血淀粉酶、尿淀粉酶的恢复时间以及住院时间均短于对照组,干预组血清总蛋白、白蛋白水平均高于对照组,干预组C-反应蛋白水平低于对照组,干预组急性生理与慢性健康量表评分低于对照组,干预组感染率与并发症发生率均低于对照组,差异有统计学意义(P<0.05);两组的病死率比较,差异无统计学意义(P>0.05)。结论SAP患者应用早期肠内营养的疗效显著,可改善临床指标,提升患者的营养水平,缓解炎症反应,减少感染与并发症,促进患者康复,值得推广应用。
Objective To observe the efficacy of early enteral nutrition(EN)on severe acute pancreatitis(SAP).Methods A total of 60 patients with SAP admitted to the Putian Medical Area,900 Hospital of the Joint Logistic Team from April 2019 to April 2021 were selected as research objects.They were divided into the control group(n=30)and the intervention group(n=30)based on the random number table method.The control group received delayed EN while the intervention group received early EN.The clinical indexes,laboratory indicators,health score,infection rate,complication rate and mortality were compared between the two groups.Results The recovery time of blood amylase,urine amylase and hospitalization time in the intervention group were shorter than those in the control group,and the levels of serum total protein and albumin in the intervention group were higher than those in the control group,while the levels of C-reactive protein in the intervention group were lower than those in the control group.Meanwhile,the scores of Acute Physiology and Chronic Health Evaluation in the intervention group were lower than those in the control group,and the infection rate and complication rate in the intervention group were lower than those in the control group,with statistically significant differences(P<0.05).There was no significant difference in mortality between the two groups(P>0.05).Conclusion Early EN is effective in patients with SAP,which can improve clinical indexes,improve the nutritional level of patients,alleviate inflammatory reaction,reduce infection and complications,and promote the rehabilitation of patients,and is worthy of popularization and application.
