摘要
目的探讨体检人群幽门螺杆菌(Hp)感染现状及与糖脂代谢、胃肠激素、miR-26a水平的关联性。方法选取2020年9-12月该院体检人群1000例,检测体检人群Hp感染情况,随机抽取72例Hp感染者作为观察组,72例Hp未感染者作为对照组。比较两组对象一般资料、糖脂代谢、胃肠激素、miR-26a水平,分析Hp感染影响因素,并比较不同Hp感染分型患者糖脂代谢、胃肠激素、miR-26a,受试者工作特征(ROC)曲线评价糖脂代谢、胃肠激素、miR-26a对Hp感染分型的鉴别。结果1000例体检者中374例Hp感染,感染率为37.40%(374/1000);两组饮酒、吸烟、外出就餐频率、家人Hp感染史比较,差异有统计学意义(P<0.05);观察组高密度脂蛋白胆固醇(HDL-C)、miR-26a低于对照组,空腹血糖(FBG)、胃动素(MTL)、胃泌素(GAS)水平均高于对照组,差异均有统计学意义(P<0.05);logistic回归分析显示,饮酒、吸烟、外出就餐频率、家人Hp感染史、FBG、HDL-C、MTL、GAS、miR-26a均为Hp感染的影响因素(P<0.05);Hp感染Ⅰ型患者HDL-C、miR-26a水平低于Ⅱ型,FBG、MTL、GAS水平高于Ⅱ型,差异均有统计学意义(P<0.05);绘制糖脂代谢、胃肠激素、miR-26a对Hp感染分型鉴别的ROC曲线,显示各指标均可用于鉴别Hp感染分型,其中miR-26a的AUC值(0.826)最大,其次是MTL、FBG、HDL-C、GAS。结论体检人群受糖脂代谢、胃肠激素、miR-26a水平、外出就餐频率等多种因素影响,Hp感染风险较高,且不同Hp分型感染患者糖脂代谢、胃肠激素、miR-26a水平不同。
Objective To explore the current status of Helicobacter pylori(Hp)infection and its correlation with glucose and lipid metabolism,gastrointestinal hormones and miR-26a levels in the physical examination population.Methods A total of 1000 people in a hospital for physical examination from September 2020 to December 2020 were selected,and the Hp infection in the physical examination population was detected.A total of 72 cases of Hp infection were randomly selected as the observation group,and 72 cases of Hp uninfected as the control group.The general data,glucose and lipid metabolism,gastrointestinal hormones and miR-26a of the two groups were compared,and the factors affecting Hp infection were analyzed.The glucose and lipid metabolism,gastrointestinal hormones and miR-26a of patients with different Hp infection types were compared.Receiver operating characteristic curve(ROC)was used to evaluate the differential value of glucose and lipid metabolism,gastrointestinal hormones and miR-26a in the classification of Hp infection.Results There were 374 cases of Hp infection among 1000 physical examination subjects(37.4%).The comparison of the frequency of drinking,smoking,eating out and family.The Hp infection history between the two groups was statistically significant(P<0.05).The HDL-C and miR-26a of the observation group were lower than those of the control group,and the levels of FBG,MTL and GAS were higher than those of the control group,and the difference was statistically significant(P<0.05).Logistic regression analysis showed that drinking,smoking,eating out frequency,family history of Hp infection,FBG,HDL-C,MTL,GAS and miR-26a were all influencing factors of Hp infection(P<0.05).HDL-C and miR-26a of typeⅠpatients with Hp infection were lower than those of typeⅡpatients,and the levels of FBG,MTL and GAS were higher than those of typeⅡpatients,and the differences were statistically significant(P<0.05).The ROC of glucose and lipid metabolism,gastrointestinal hormones and miR-26a in the differential diagnosis of Hp infection typing was drawn,showing that each index can be used for differential diagnosis of Hp infection typing.The AUC value of miR-26a of 0.826 was the largest,followed by MTL,FBG,HDL-C and GAS.Conclusion The physical examination population is affected by various factors such as glucose and lipid metabolism,gastrointestinal hormones,miR-26a level and frequency of eating out.The risk of H.pylori infection is higher,and the levels of glucose and lipid metabolism,gastrointestinal hormones and miR-26a are different in patients with different H.pylori type infections.
