摘要
目的探讨经侧后路椎间孔镜下椎间盘髓核摘除术(PTED)对腰椎间盘突出症患者术后疼痛程度(VAS)及腰椎功能(ODI)的影响。方法选取腰椎间盘突出症患者82例,依据随机数字表法分为研究组(n=41)及对照组(n=41)。对照组采取后路椎板间开窗髓核摘除术(FD)术式,研究组采取PTED。统计两组手术情况、手术前后VAS及ODI评分、血清可溶性肿瘤坏死因子I型受体(sTNF-RI)、前列腺素E2(PGE2)水平、并发症。结果研究组手术时长长于对照组,但切口长度小于对照组,术中失血量少于对照组,术后卧床时长、术后住院时长短于对照组(P<0.05);术后研究组VAS、ODI评分低于对照组(P<0.05);术后1 d研究组血清sTNF-RI水平高于对照组,PGE2水平低于对照组(P<0.05);研究组并发症发生率(4.88%)低于对照组(19.51%)(P<0.05)。结论采取PTED治疗腰椎间盘突出症效果显著,且具有安全性,利于改善患者腰椎功能。
Objective To investigate the effect of intervertebral disc nucleus pulposus excision under lateral posterior foraminal endoscopy(PTED)on postoperative pain(VAS)and lumbar function(ODI)in patients with lumbar disc herniation.Methods 82 patients with lumbar disc herniation in our hospital were selected and divided into a study group(n=41)and a control group(n=41)according to the random number table method.The control group was treated withposterior interlaminar fenestration of the nucleus pulposus(FD)surgery,and the study group was treated with PTED.The operation condition,VAS and ODI scores before and after operation,serum soluble tumor necrosis factor type I receptor(sTNF-RI),prostaglandin E2(PGE2)levels,and complications were counted.Results The operation time of the study group was longer than that of the control group,but the incision length was shorter than that of the control group,The blood loss during operation was less than that of the control group,the length of postoperative bed stay and postoperative hospital stay in the study group were shorter than those in the control group(P<0.05);The VAS and ODI scores of the study group after operation were lower than those of the control group(P<0.05);The serum sTNF-RI level of the study group was higher than that of the control group,and the PGE2 level was lower than that of the control group 1 day after operation(P<0.05);The complication rate of the study group(4.88%)was lower than that of the control group(19.51%)(P<0.05).Conclusion The treatment of lumbar disc herniation with PTED has significant effect and safety,which is beneficial to improve the lumbar function of patients.
