摘要
目的:分析本院骨科患者围术期使用质子泵抑制剂(PPIs)预防应激性溃疡(SU)的合理性,为临床合理使用PPIs提供参考。方法:通过随机抽取2020年1月~12月本院骨科出院患者中围术期使用过PPIs的病例500例,统计患者基本情况、PPIs使用情况、手术情况等,综合分析PPIs使用的合理性。结果:在500例患者中,408例(81.6%)具备预防SU的用药指征,92例(18.4%)无预防SU的用药指征;35例(7.0%)患者给药频次不合理,12例(2.4%)患者给药剂量较大,43例(8.6%)患者给药疗程较长(>7天)。涉及的手术类型主要为骨折内固定术,颈腰椎融合术、全髋关节置换术、全膝关节置换术、人工股骨头置换术等。500例使用PPIs的患者中,412例(82.4%)患者使用静脉用PPIs,88例(17.6%)患者使用口服用的PPIs。结论:本院骨科围术期患者PPIs在用药指征、给药方式、用药疗程、药物品种选择等方面存在一些不合理现象,需采取相应标准或措施来促进该类药物的合理使用。
Objective:To analyze the rationality of using proton pump inhibitors(PPIs)to prevent stress ulcer(SU)in orthopedic patients during perioperative period in our hospital,so as to provide references for clinical rational use of PPIs.Methods:A total of 500 cases of patients discharged from orthopedics department of our hospital from January to December in 2020 who had used PPIs during the perioperative period were randomly selected.The basic situation of patients,the use of PPIs and the operation situation were analyzed,and the rationality of PPIs use was comprehensively analyzed.Results:Among the 500 patients,408 patients(81.6%)had medication indications for preventing SU;92 patients(18.4%)had no medication indications for preventing SU;35 patients(7.0%)had unreasonable administration frequency,12 patients(2.4%)had large dosage,and 43 patients(8.6%)had long course of administration(>7 days).The main types of operation involved are fracture internal fixation,cervical and lumbar fusion,total hip replacement,total knee replacement,artificial femoral head replacement,etc.Of the 500 patients who used PPIs,412 patients(82.4%)used intravenous PPIs and 88 patients(17.6%)used oral PPIs.Conclusion:There are some unreasonable phenomena of PPIs in orthopedic perioperative patients in our hospital,such as medication indication,mode of administration,medication course,drug selection,and so on.It is necessary to take corresponding standards or measures to promote the rational use of such drugs.
