摘要
目的探讨孕早期稽留流产患者发生的相关风险因素。方法收集2018年10月—2020年6月在邵阳学院附属第一医院妇产科住院的1047例孕产妇,其中489例孕≤12周稽留流产患者为试验组,558例正常妊娠分娩的孕妇作为对照组。回顾性分析两组患者的一般临床资料,并分析影响稽留流产发生的相关风险因素。结果经单因素分析显示,试验组患者的年龄比对照组高,被动吸烟、意外妊娠人数比对照组多,脱畸检查与主动回避动物的人数比对照组少(P<0.05);试验组患者接触的电离辐射与毒害物的人数比对照组多;多因素分析显示,高龄妊娠(≥35岁)、被动吸烟、未做脱畸检查是稽留流产发生的独立危险因素(P<0.05)。结论高龄、被动吸烟的患者发生稽留流产的风险增加,对于此类患者需积极行产前检查及诊断,并积极干预。
Objective To investigate risk factors of missed abortion in early pregnancy.Methods 1047 cases of parturients who treated at department of obstetrics and gynecology,the First Affiliated Hospital of Shaoyang University from October 2018 to June 2020 were selected,randomly divided into two groups according the delivery outcome,489 cases of parturients with missed abortion of no more than 12 weeks gestation as the test group,and 558 cases of parturients with no adverse pregnancy outcomes as the control group,the general data of the two groups were investigated,and the relevant factors affecting the occurrence of adverse pregnancy outcomes were analyzed.Results According to single factor analysis shows,in the test group,the age of the pregnant women was higher than the control group,and the number of passive smoking and unexpected pregnancy was higher than the control group,the number of prolapse examinations and active avoidance of animals were less than the control group(P<0.05).In the test group,the number of parturients who were exposed to ionizing radiation and toxic substances were more than the control group.Multi-factor analysis shows,older pregnancy(≥35 years old),passive smoking,and no malformation test were the independent risk factors for adverse pregnancy outcomes(P<0.05).Conclusion The subgroup of patients characterized by old age,passive smoking have a higher risk of missed abortion.Prenatal examination and diagnosis should be performed actively for such patients.
