摘要
目的探究肺癌患者家庭功能与死亡焦虑的关系及生命意义寻求和体验在两者间的中介作用。方法 2019年6月至2020年11月,便利抽样法选取郑州市两所三级甲等医院呼吸内科和肿瘤科住院肺癌患者361例为研究对象。采用一般资料调查表、家庭关怀度指数问卷、生命意义感量表、死亡焦虑量表对其进行调查。结果肺癌患者家庭功能总分、生命意义总分、生命意义寻求、生命意义体验及死亡焦虑得分分别为(6.90±1.56)分、(42.34±4.58)分、(19.83±2.25)分、(22.51±2.33)分、(7.13±2.12)分。家庭功能与生命意义寻求、生命意义体验得分呈正相关、与死亡焦虑得分呈负相关(均P<0.01)。家庭功能对死亡焦虑的作用路径包括"家庭功能→生命意义寻求→死亡焦虑"、"家庭功能→生命意义体验→死亡焦虑"和"家庭功能→生命意义寻求→生命意义体验→死亡焦虑",其中生命意义追寻和体验的链式中介效应占总效应的45.10%。结论家庭功能不仅可通过生命意义寻求、生命意义体验的中介作用缓解肺癌患者的死亡焦虑,还可通过生命意义寻求-体验的链式中介作用间接影响死亡焦虑。
Objective To explore the relationship between family function and death anxiety in patients with lung cancer and the mediating role between search for meaning and presence of meaning.Methods From June 2019 to November 2020,361 inpatients with lung cancer of two hospitals in Zhengzhou were selected by convenience sampling method and investigated by general data questionnaire, Family APGAR Index, Meaning in Life Question and Templer’s Death Anxiety Scale.Results The scores of family function, meaning of life, search for meaning, presence of meaning and death anxiety were(6.90±1.56),(42.34±4.58),(19.83±2.25),(22.51±2.33),and(7.13±2.12),respectively.Correlation analysis showed that family function, search for meaning and presence of meaning were positively correlated while family function was negatively correlated with death anxiety(all P<0.01).The pathway of family function to death anxiety included “family function→search for meaning→death anxiety”,“family function→presence of meaning→death anxiety” and “family function→search for meaning→ presence of meaning→death anxiety”.The chain mediating effect accounted for 45.10% of the total effect.Conclusions Family function can release the death anxiety through the mediating role of search for meaning and presence of meaning.The chain mediating role of “search for meaning→presence of meaning” can directly affect the death anxiety.
