摘要
目的探讨腹腔镜联合胆道镜与传统开腹手术治疗胆囊结石合并胆总管结石的临床效果。方法将2016年6月—2021年6月该院收治的60例胆囊结石合并胆总管结石患者,按随机数字表法分为研究组和对照组,各30例。研究组采取腹腔镜胆囊切除+胆总管切开+胆道镜探查取石、T管引流术;对照组采取传统开腹手术。比较两组患者的临床治疗效果。结果研究组术中失血量(56.5±14.2)mL、术后恢复排气时间(31.9±8.0)h、住院时间(6.4±1.5)d均低于对照组,两组比较差异有统计学意义(t=-7.472、-13.598、-2.972,P<0.05);两组手术时间比较差异无统计学意义(P>0.05);术后并发症发生率3.3%低于对照组的13.3%,两组比较差异有统计学意义(χ^(2)=4.286,P<0.05)。结论腹腔镜联合胆道镜治疗胆囊结石合并胆总管结石可减少术中失血量,创伤小、恢复快,同时还可以使术后疼痛程度、并发症发生率、住院时间降低或减少,对于患者病情恢复有益。
Objective To investigate the clinical effect of laparoscopy combined with choledochoscope and traditional laparotomy in the treatment of cholecystolithiasis with common bile duct stones.Methods A total of 60 patients with cholecystolithiasis and common bile duct stones admitted from June 2016 to June 2021 in the hospital were divided into study group and control group by random number table method,30 cases each.The study group adopted laparoscopic cholecystectomy+choledocholithotomy+choledochoscopy for stone extraction and T tube drainage.The control group underwent traditional open surgery.Compared the clinical treatment effects of the two groups of patients.Results The blood loss in the study group was(56.5±14.2)mL,the postoperative recovery time was(31.9±8.0)h,and the hospitalization time was(6.4±1.5)d,which was all lower than those of the control group,the difference between the two groups was statistically significant(t=-7.472,-13.598,-2.972,P<0.05).The difference in operation time between the two groups was not statistically significant(P>0.05);the postoperative complication rate of the study group was 3.3%,which was lower than the control group's 13.3%,the difference between the groups was statistically significant(χ^(2)=4.286,P<0.05).Conclusion Laparoscopy combined with choledochoscopy for the treatment of gallbladder stones combined with choledocholithiasis can reduce intraoperative blood loss,with less trauma and faster recovery.At the same time,it can reduc e or reduce postoperative pain,complication rate,and hospital stay,which is beneficial to the recovery of patients.
