摘要
目的分析卵巢囊肿患者腹腔镜术后发生肠麻痹的影响因素。方法选取淅川县人民医院2018年6月至2020年6月收治的40例接受腹腔镜手术后发生肠麻痹的卵巢囊肿患者纳入发生组;另选取医院同期收治的40例接受腹腔镜手术后未发生肠麻痹的卵巢囊肿患者纳入未发生组。统计两组患者临床资料,经多因素分析卵巢囊肿患者腹腔镜术后发生肠麻痹的影响因素。结果发生组盆腔粘连患者多于未发生组,手术时间长于未发生组,血清白细胞介素(IL)-6、可溶性细胞间黏附分子(sICAM)-1、肿瘤坏死因子(TNF)-α水平高于未发生组,差异有统计学意义(P<0.05)。经logistic回归分析显示,盆腔粘连、手术时间长、血清IL-6、sICAM-1、TNF-α过表达是卵巢囊肿患者腹腔镜术后发生肠麻痹的影响因素(OR>1,P<0.05)。结论盆腔粘连、手术时间长、血清IL-6、sICAM-1、TNF-α过表达是卵巢囊肿患者腹腔镜术后发生肠麻痹的影响因素。
Objective To analyze the influencing factors of intestinal paralysis after laparoscopic surgery in patients with ovarian cyst.Methods The clinical data of 40 patients with ovarian cysts with intestinal paralysis after laparoscopic surgery treated in Xichuan People’s Hospital from June 2018 to June 2020 were collected and included in the occurrence group;in addition,the clinical data of 40 patients with ovarian cyst without intestinal paralysis after laparoscopic surgery in the hospital during the same period were collected and included in the non occurrence group.Relevant data of patients in the two groups were counted,the influencing factors of intestinal paralysis after laparoscopic surgery in patients with ovarian cyst were analyzed by multivariate analysis.Results The proportion of patients with pelvic adhesion in the occurrence group was more than that in the non occurrence group,the operation time was longer than that in the non occurrence group,the levels of serum interleukin(IL)-6,soluble intercellular adhesion molecule(sICAM)-1 and tumor necrosis factor(TNF)-αwere higher than those in the non occurrence group(P<0.05).Logistic regression analysis showed that pelvic adhesion,long operation time,overexpression of serum IL-6,sICAM-1 and TNF-αwere the influencing factors of intestinal paralysis in patients with ovarian cyst after laparoscopic surgery(OR>1,P<0.05).Conclusion Pelvic adhesion,long operation time,overexpression of serum IL-6,sICAM-1 and TNF-αare the influencing factors of intestinal paralysis in patients with ovarian cyst after laparoscopic surgery.
