摘要
目的探究疏肝健脾方对桥本甲状腺炎患者甲状腺功能和免疫炎性反应的影响。方法将我院收治的82例桥本甲状腺炎患者随机分为对照组和治疗组。对照组进行常规西药治疗;治疗组在对照组的基础上给予疏肝健脾方治疗,治疗周期为4周。比较两组患者免疫炎性因子水平(IFN-γ、TNF-α、IL-23、IL-6)、甲状腺功能指标(FT3、FT4、TSH、TGAb、TPOAb)、中医证候积分及临床疗效。结果治疗后,两组患者FT3、FT4均升高,TSH、TGAb及TPOAb均降低(P<0.05),治疗组患者上述指标改善均优于对照组(P<0.05);两组患者IL-6、IL-8、IFN-γ、TNF-α均有所改善,与对照组比较,治疗组上述指标降低程度更加明显(P<0.05);治疗后两组患者中医证候积分均降低,且治疗组低于对照组(P<0.05);治疗组临床总有效率明显高于对照组(95.12%vs.70.73%,P<0.05)。结论疏肝健脾方为指导原则的中药煎剂在治疗桥本甲状腺炎患者方面取得较为满意的临床疗效,可明显缓解桥本甲状腺炎患者临床症状,改善甲状腺功能,提高临床疗效。
Objective To investigate the effect of Shugan Jianpi Formula on thyroid function and immune inflammatory response in Hashimoto thyroiditis patients.Methods Totally 82 patients with Hashimoto thyroiditis admitted to our hospital were randomly divided into control group and treatment group.Control group was treated with western medicine;patients in treatment group were given Shugan Jianpi Formula on the basis of treatment for control group.Both groups were treated for 4 weeks.The level of immune inflammatory factors such as interferon-γ(IFN-γ),tumor necrosis factor-α(TNF-α),interleukin-6(IL-6),interleukin-23(IL-23)and the contents of thyroid functional indexes such as free triiodothyronine(FT3),free tetraiodothyronine(FT4),thyroid stimulating hormone(TSH),anti-thyroglobulin antibodies(TGAb)and anti-peroxidase antibody(TPOAb),as well as the TCM syndrome score and clinical efficacy were compared between the two groups.Results After treatment,the levels of FT3 and FT4 in the two groups were increased(P<0.05),while the levels of TSH,TGAb and TPOAb were decreased(P<0.05).Compared with the control group,the decrease in treatment was more significant.After treatment,the levels of IL-8,IFN-γand TNF-αin the two groups were decreased(P<0.05),and those in treatment group was lower than control group(P<0.05).After treatment,the TCM syndrome score of the two groups was decreased(P<0.05),and treatment group was lower than control group(P<0.05).The total effective rate of treatment group was higher than that of control group(95.12%vs.70.73%,P<0.05).Conclusion The TCM decoction guided by Shugan Jianpi Formula has good clinical effects in the treatment of Hashimoto thyroiditis.It can effectively relieve the clinical symptoms of patients with Hashimoto thyroiditis,improve the thyroid function and clinical efficacy.
