摘要
目的:探讨白细胞介素17(IL-17)、血管内皮生长因子(VEGF)和肿瘤坏死因子(TNF-α)在先天性白内障摘除术后无晶状体眼青光眼(AG)患儿房水的表达水平,并分析其与疾病进程的相关性。方法:150例先天性白内障摘除手术未植入人工晶状体患儿根据术后随访期内是否发生青光眼分为青光眼组(AG组,n=56例)和非青光眼组(非AG组,n=94例),比较两组随访期间房水中IL-17、VEGF、TNF-a表达水平和眼压情况。结果:术中,AG组和非AG组房水IL-17、VEGF、TNF-α水平比较,差异无统计学意义(P>0.05);术后4个月、8个月和12个月,AG组房水IL-17、VEGF、TNF-α水平与术中比较明显升高,且高于对照组(P<0.05);术后4个月、8个月和12个月,AG组眼压高于非AG组各时间点的眼压,差异有统计学意义(P<0.05);Pearson相关性分析表明,AG组术后4个月、8个月和12个月房水IL-17、VEGF、TNF-α水平与眼压呈正相关(P<0.05);受试者工作曲线(ROC)分析术后4个月房水IL-17、VEGF、TNF-α对无晶状体眼青光眼患者的诊断价值发现,IL-17、VEGF、TNF-α的ROC曲线下面积(AUC)分别为0.714,0.708,0.724,三者联合检测的AUC值为0.792。结论:房水IL-17、VEGF和TNF-α可作为无晶状体眼青光眼的潜在的生物标志物。
Objective:To investigate the expression levels of interleukin-17(IL-17),vascular endothelial growth factor(VEGF)and tumor necrosis factor(TNF-α)in patients with aphakia glaucoma(AG)after congenital cataract extraction,and to analyze their correlation.Methods:150 children with congenital cataract extraction without intraocular lens implantation were divided into glaucoma group(AG group,n=56)and non-glaucoma group(non-AG group,n=94)according to the occurrence of glaucoma during the follow-up period.The expression levels of IL-17,VEGF and TNF-αin aqueous humor and intraocular pressure were compared between the two groups during the follow-up period.Results:There were no significant differences in the levels of IL-17,VEGF and TNF-αin intraoperative aqueous humor between the two groups(P>0.05).The levels of IL-17,VEGF and TNF-αin aqueous humor of AG group at 4,8 and 12 months after surgery were significantly increased,and the AG group was higher than the control group(P<0.05).The IOP of AG group at 4,8 and 12 months after surgery was significantly higher than that of non-AG group,and the difference was statistically significant(P<0.05).Pearson correlation analysis showed that the levels of IL-17,VEGF and TNF-αin the fluid of the AG group were positively correlated with IOP at 4,8 and 12 months after surgery(P<0.05).ROC curve analysis of the diagnostic value of IL-17,VEGF and TNF-αin patients with glaucoma 4 months after surgery showed that the AUC value of IL-17,VEGF and TNF-αin the fluid was 0.714,0.708 and 0.724,respectively,and the AUC value of the combined detection of the three was 0.792.Conclusion:Aquatic fluid IL-17,VEGF and TNF-αcan be used as potential biomarkers for glaucoma in aphageal eyes.
