摘要
目的观察化腐再生法对糖尿病伴肌腱坏死大鼠创周组织神经生长因子(nerve growth factor,NGF)及其高亲和力受体酪氨酸激酶A(tyrosine kinase receptor A,TrkA),神经肽P物质(substance P,SP)表达的影响及对筋之血化过程的影响。方法30只大鼠随机分为模型组、对照组、治疗1组、治疗2组、治疗3组。5组均腹腔注射链脲佐菌素(0.35 mg/100 g)建立糖尿病模型,后采用外科方法及25%冰醋酸50U外滴创面,建立糖尿病大鼠肌腱变性坏死溃疡模型。采用凡士林油纱、水凝胶、生肌象皮膏、菠萝蛋白酶等不同药物外敷治疗。分别于治疗3、7、14 d提取大鼠创周组织制成切片,免疫组化染色,采用显微镜结合光密度检测法分析不同时期NGF、TrkA及SP的阳性表达。结果治疗期间对照组、治疗1、2、3组创周组织NGF、TrkA及SP水平均呈先上升后下降趋势。3 d时治疗3组创周组织NGF、TrkA及SP水平均高于模型组(P<0.01),7 d时治疗3组创周组织NGF及SP水平高于模型组(P<0.05,P<0.01),14 d时治疗3组创周组织NGF水平低于模型组(P<0.05)。结论化腐再生法能够在不同时期调控糖尿病伴肌腱坏死大鼠血创周组织NGF、TrkA与SP水平,促进筋之血化过程,有利于创面愈合。
Objective To observe the influence of resolving rotten tissue and regeneration method(A kind of treatment of liquefying necrotic tissues and promoting granulation for chronic ulcer)on expressions of nerve growth factor(NGF),tyrosine kinase A(TrkA)and substance P(SP),and the process of Jinzhixuehua in diabetic rats with tendon necrosis.Methods Thirty rats were randomly divided into model group,control group,treatment group 1,treatment group 2 and treatment group 3.The diabetic rat model with tendon necrosis was established by intraperitoneal injection of streptozotocin(0.35mg/100g).The surgical method and external use of 25%glacial acetic acid 50U were used in each group.And then the rats were treated externally with vaseline gauze,hydrogel,Shengji plaster and bromelain,respectively.The samples of wound were collected for making into slices after treatment for 3,7 and 14 days,and were given immunohistochemistry staining.The expressions,changes and influence of NGF,TrkA and SP were detected and analyzed by using microscopy and average optical(AO)detection.Results During the treatment,the expressions of NGF,TrkA and SP in the control group,treatment 1,treatment 2 and treatment 3 groups increased first and then decreased.In treatment group 3,the expressions of NGF,TrkA and SP were higher than those in the model group on the 3rd day(P<0.01).The expressions of NGF and SP were higher than those in the model group on the 7th day(P<0.05,P<0.01),and the expression of SP was lower than that in the model group on the 14th day(P<0.05).Conclusion The resolving rotten tissue and regenerationmethod can regulate the expressions of NGF,TrkA and SP in the wound of diabetic rats with tendon necrosis,and promote the process of blood regeneration of tendons,which is good for wound healing.
