摘要
目的探讨胰腺导管腺癌(pancreatic ductal adenocarcinoma,PDAC)大切片中T淋巴细胞相关抗原CD8的表达与临床病理特征及预后的关系。方法收集57对术前未行新辅助治疗并经外科手术切除的PDAC石蜡样本大、小切片,采用免疫组化EnVision法检测CD8在大、小切片组的表达,分析其表达与PDAC临床病理特征和预后的关系。结果CD8在PDAC大、小切片组中均有表达,大切片组中CD8的表达显著高于小切片组(P<0.05)。大切片组CD8高表达与患者性别、年龄、肿瘤部位、肿瘤直径、肿瘤分化程度、pTNM分期、是否有胰周淋巴结转移、总淋巴结转移数目、神经侵犯、脉管癌栓均无明显相关性(P>0.05),小切片组CD8高表达与肿瘤分化程度相关(P=0.036)。Cox多因素分析显示,影响PDAC预后的独立因素有4个:年龄(P=0.002)、肿瘤分化程度(P=0.012)、总淋巴结转移数(P=0.003)、胰腺癌大切片中CD8表达(P<0.001)。结论PDAC大切片中CD8高表达更能提示患者预后良好,CD8有望成为胰腺癌治疗的潜在靶标。
Purpose To investigate the relationship between the expression of T lymphocyte associated antigen CD8 in pancreatic ductal adenocarcinoma(PDAC)based on large sections of histopathology(LSH)and its clinicopathological features and prognosis.Methods 57 pairs of LSH and small sections of histopathology(SSH)of PDAC paraffin samples were collected.They had not undergone neoadjuvant therapy before surgery and had been surgically resected.Immunohistochemistry EnVision staining method was used to detect the expression of CD8 in LSH and SSH of PDAC,and then to analyze whether CD8 and clinicopathological characteristics correlated with clinical prognosis.Results Both LSH and SSH successfully detected cell expressing CD8.The expression of CD8 in LSH was significantly higher than that SSH(P<0.05).There was no significant correlation between the high expression of CD8 in LSH and the gender,age,tumor location,tumor diameter,tumor differentiation,pTNM stage,peripancreatic lymph node metastasis,total number of lymph node metastases,nerve invasion,and vascular tumor thrombus(P>0.05).However,the high expression of CD8 in SSH was correlated with tumor differentiation(P=0.036).Cox multivariate analysis showed that there were 4 independent factors affecting the prognosis of PDAC:age(P=0.002),tumor differentiation(P=0.012),total number of lymph node metastases(P=0.003),and the expression of CD8 in LSH(P<0.001).Conclusion High expression of CD8 in LSH of PDAC can better indicate that patients have a good prognosis,and CD8 may become a potential target for pancreatic cancer treatment.
