摘要
目的研究他克莫司联合泼尼松治疗肾病综合征(NS)的临床疗效。方法以2017年6月~2019年12月宣城市人民医院肾内科收治的90例NS患者为研究对象,依据简单随机数字表法分为研究组与对照组,各45例。对照组采取泼尼松(服用24周),研究组在对照组基础上加用他克莫司(服用24周)。统计两组临床疗效,取血液样本、尿液样本检测治疗前后肾功能指标24 h尿蛋白定量(24 hPro)、血清肌酐(Scr)、尿素氮(BUN)、白蛋白水平、血清可溶性尿激酶型纤溶酶原激活物受体(suPAR)、可溶性髓样细胞触发受体(sTREM-1)水平、补体C3水平、不良反应发生率。结果与对照组(75.56%)比较,研究组总有效率(93.33%)增高,差异有统计学意义(P<0.05)。与对照组比较,治疗后研究组24 hPro、Scr、BUN水平降低,白蛋白水平增高,血清suPAR、sTREM-1水平降低,补体C3水平增高,差异均有统计学意义(P<0.05)。两组不良反应发生率差异无统计学意义(P>0.05)。结论他克莫司联合泼尼松治疗NS可有效改善患者肾功能,调节suPAR、sTREM-1、补体C3水平,提高整体治疗效果。
Objective To study the clinical effect of tacrolimus combined with prednisone in nephrotic syndrome(NS).Methods A total of 90 patients with NS admitted to the Department of Nephrology in Xuancheng people's Hospital from June 2017 to December 2019 were divided into study group and control group randomly,with 45 cases in each group.The control group was treated with prednisone for 24 weeks,while the study group was treated with tacrolimus for 24 weeks on the basis of the control group.Blood and urine samples were detected with the levels of 24 h urinary protein(24hpro),serum creatinine(SCR),urea nitrogen(BUN)and albumin,serum soluble urokinase type plasminogen activator receptor(suPAR),soluble triggering receptor of myeloid cells(sTREM-1),complement C3.And the incidence of adverse reactions were observed.Results The total effective rate of the study group(93.33%)was higher than that of the control group(75.56%)(P<0.05).Compared with the control group,24 hpro,SCR,BUN levels,albumin levels,serum suPAR,srem-1 levels were decreased,complement C3 levels increased,and the differences were statistically significant(all P<0.05).There was no significant difference in the incidence of adverse reactions between the two groups(P>0.05).Conclusion Tacrolimus combined with prednisone in the treatment of NS can effectively improve the renal function,regulate the levels of suPAR,sTREM-1 and complement C3,and improve the overall therapeutic effect.
