摘要
目的探讨磁共振成像(MRI)动态增强扫描在脊柱结核与脊柱转移瘤鉴别中的诊断价值。方法选取湖南省胸科医院2017年12月—2019年12月收治的经病理检查确诊的脊柱结核患者100例作为观察组,同期收治经病理检查确诊的脊柱转移瘤患者100例作为对照组。比较2组IVIM参数,包括灌注分数(F)、准扩散系数(D*)、扩散系数(D),并比较2组噪声比(CNR)、表观扩散系数(ADC)及影像学特征。结果观察组F高于对照组,D*、D低于对照组,CNR、ADC低于对照组(P<0.05)。2组病变部位、病灶数量比较,差异无统计学意义(P>0.05);观察组累积椎间盘、椎体轮廓压缩变形、软组织病变信号不均匀、病灶形态长条梭形发生率高于对照组(P<0.05)。结论 MRI动态增强扫描可准确对脊柱结核、脊柱转移瘤做出诊断并加以鉴别,能为临床治疗方案的制定提供影像学参考依据。
Objective To investigate the diagnostic value of dynamic contrast-enhanced MRI in differentiating spinal tuberculosis from spinal metastases.Methods A total of 100 cases of patients with spinal tuberculosis confirmed by pathological examination were selected from December 2017 to December 2019 in Hunan Chest Hospital,which were selected as the observation group,and 100 patients with spinal metastases confirmed by pathological examination were selected as the control group.IVIM parameters including perfusion fraction(F),quasi diffusion coefficient(D*),diffusion coefficient(D),noise ratio(CNR),apparent diffusion coefficient(ADC)and imaging features were compared between the two groups.Results F of observation group was higher than that of control group,D*,D were lower than that of control group,CNR,ADC were lower than that of control group(P<0.05).There was no significant difference in lesion location and number between the two groups(P>0.05);the incidence of cumulative intervertebral disc,vertebral contour compression deformation,uneven signal of soft tissue lesions and long fusiform lesions in the observation group was higher than that in the control group(P<0.05).Conclusion Dynamic contrast-enhanced MRI can accurately diagnose and differentiate spinal tuberculosis and spinal metastases,and can provide imaging reference for clinical treatment.
