摘要
目的探讨连续收肌管阻滞联合坐骨神经阻滞对膝关节置换术后疼痛、应激反应及关节功能的影响。方法选取湘南学院附属医院2019年2月—2020年2月收治的老年膝关节置换术后患者102例,依据镇痛方式不同分为参照组与联合组,各51例。参照组予以连续收肌管阻滞,联合组在参照组基础上行坐骨神经阻滞。比较2组术后1 d、3 d、5 d视觉模拟评分法(VAS)评分、C反应蛋白(CRP)水平,术后7 d、1个月关节活动度,并观察2组不良反应发生情况。结果术后1 d、3 d,联合组VAS评分、CRP水平低于参照组(P<0.05)。术后5 d,2组VAS评分、CRP水平比较,差异无统计学意义(P>0.05)。术后7 d、1个月联合组关节活动度高于参照组(P<0.05)。2组不良反应发生率比较,差异无统计学意义(P>0.05)。结论连续收肌管阻滞联合坐骨神经阻滞在膝关节置换术后患者中的应用效果良好,可减轻患者疼痛,改善应激反应,有助于促进关节功能恢复,且安全性较高。
Objective To explore the impact on continuous adductor canal block combined with sciatic nerve block on pain,stress response and joint function after knee replacement.Methods A total of 102 cases of elderly patients after knee replacement were selected from February 2019 to February 2020 in the Affiliated Hospital of Xiangnan University,which were divided into reference group and combined group according to different analgesia methods,with 51 cases in each group.The reference group was given continuous adductor canal block,the combined group received sciatic nerve block on the basis of the control group.The VAS score and CRP level on the 1 st,3 rd and 5 th day after operation,and the range of motion on 7 days and 1 month after operation were compared between the two groups,and the incidence of adverse reactions were observed.Results On the 1 st and 3 rd day after operation,the VAS score and CRP level of the combined group were lower than control group(P<0.05).5 days after operation,there was no significant difference in VAS score and CRP level between the two groups(P>0.05).On 7 days and 1 month after operation,the range of motion in the combined group was higher than control group(P<0.05).There was no significant difference in the incidence of adverse reactions between the two groups(P>0.05).Conclusion Continuous adductor block combined with sciatic nerve block has good analgesic effect in patients after knee arthroplasty,which can reduce pain,improve stress response,promote joint function recovery,and has high safety.
