摘要
目的探讨个性化镇痛泵(IAP)对全膝关节置换术(TKA)后患者镇痛药使用及膝关节功能的影响。方法回顾性分析88例行TKA治疗的膝关节骨性关节炎患者临床资料,按术后镇痛方式分为观察组60例及对照组28例,观察组术后使用IAP镇痛,对照组术后不使用IAP镇痛。比较两组患者术后镇痛药物使用次数、功能康复依从性及膝关节外科协会(KSS)评分。结果观察组术后镇痛药使用次数、肛门用药次数及口服/外用药使用次数均多于对照组(P<0.05),两组患者术后静脉/肌注镇痛药次数比较,差异无统计学意义(P>0.05)。观察组患者术后功能康复依从性评分、KSS总分、临床疼痛评分及活动度均高于对照组(P<0.05),但两组术后膝关节功能的优良率比较,差异无统计学意义(P>0.05)。结论 TKA术后使用IAP可提高患者功能锻炼的依从性及膝关节活动度,但会一定程度增加镇痛药使用次数。
Objective To investigate the effects of individual analgesic pump（ IAP） on the analgesics use and knee function after total knee arthroplasty（ TKA）. Methods The clinical data of 88 patients with knee osteoarthritis undergoing TKA were retrospectively analyzed. According to the approaches to postoperative analgesia,the patients were divided into observation group（ 60 cases） and control group（ 28 cases）. The observation group was given analgesia with IAP after operation,while the control group was given analgesia without IAP after operation. The frequency of analgesics use,functional rehabilitation compliance and Knee Surgery Society（ KSS） score were compared between the two groups after operation. Results After operation,the total frequency of analgesics use,number of anal medication,frequency of oral/topical use of analgesics in the observation group were all higher than those in the control group（ P〈0. 05）,and there was no significant difference in the frequency of intravenous/intramuscular injection of analgesics between the two groups（ P〉 0. 05）. The postoperative functional rehabilitation compliance score,KSS total score,clinical pain score and range of motion in the observation group were higher than those in the control group（ P 〈0. 05）,but there was no significant difference in the excellent and good rate of postoperative knee function between the two groups（ P〉 0. 05）. Conclusion IAP use after TKA can improve the patients＇ compliance of functional exercise and range of knee motion,but will increase the frequency of analgesics use to a certain extent.
