短期重组人生长激素治疗生长激素缺乏症儿童的效果分析 被引量:6

摘要 目的探讨生长激素缺乏症(growth hormone deficiency,GHD)儿童短期使用重组人生长激素(recombinant human growth hormone,rhGH)治疗的效果及对生长速率(growth rate,GV)、胰岛素样生长因子-1(insulin-like growth factor-1,IGF-1)等的影响。方法选取2014年1月至2019年1月在本院治疗的GHD儿童80例,男49例,女31例,年龄(9.10±2.03)岁。给予短期rhGH治疗,检测治疗前后身高、GV、IGF-1等指标。结果治疗后1个月、3个月患儿身高、GV及身高标准差积分(HtSDS)分别为(123.51±9.88)cm和(128.82±8.10)cm、(5.60±1.02)cm/y和(11.62±2.01)cm/y、(-2.26±0.45)分和(-2.10±0.34)分,均较治疗前升高(均P<0.05);治疗后1个月、3个月患儿IGF-1、空腹胰岛素(fasting insulin,FINS)、空腹血糖(fasting glucose,FBG)和游离三碘甲状腺原氨酸(free triiodinated thyroxine,FT3)分别为(130.29±45.41)μg/L和(210.01±89.28)μg/L、(7.52±1.33)μU/ml和(8.30±2.01)μU/ml、(5.08±0.67)mmol/L和(5.10±0.70)mmol/L、(4.15±0.65)ng/L和(4.20±0.70)ng/L,均明显高于治疗前(均P<0.05);HtSDS与身高、GV、IGF-1呈正相关(r=0.384、0.390、0.362,均P<0.05),GV与IGF-1呈正相关(r=0.301,P<0.05);多元线性逐步回归分析结果显示:治疗前身高、IGF-1、HtSDS均是GV的影响因素(均P<0.05)。结论GHD儿童短期使用rhGH治疗效果较好,治疗前身高、IGF-1及HtSDS是患儿治疗效果的影响因素。 Objective To investigate the effect of short-term recombinant human growth hormone(rhGH)in the treatment of children with growth hormone deficiency(GHD)and its effects on growth rate(GV)and insulin-like growth factor-1(IGF-1).Methods Eighty children with GHD in our hospital fromJanuary 2014 to January 2019were selected,including 49males,31 females,aged(9.10±2.03)years old,and short-term rhGH treatment was given,the height,GV,IGF-1,and other indicators before and after treatment were measured.Results The height,GV,and height standard deviation score(HtSDS)of the children at 1 and 3 months after treatment were(123.51±9.88)cm and(128.82±8.10)cm,(5.60±1.02)cm/y and(11.62±2.01)cm/y,(-2.26±0.45)and(-2.10±0.34),which were higher than those before treatment(all P<0.05);the levels of IGF-1,fasting insulin(FINS),fasting glucose(FBG),and free triiodinated thyroxine(FT3)at 1 and 3 months after treatment were(130.29±45.41)μg/L and(210.01±89.28)μg/L,(7.52±1.33)μU/ml and(8.30±2.01)μU/ml,(5.08±0.67)mmol/L and(5.10±0.70)mmol/L,(4.15±0.65)ng/L and(4.20±0.70)ng/L,which were significantly higher than those before treatment(all P<0.05).HtSDS was positively correlated with height,GV,IGF-1(r=0.384,0.390,and 0.362,all P<0.05),GV was positively correlated with IGF-1(r=0.301,P<0.05).The results of multiple linear stepwise regression analysis showed that height,IGF-1,and HtSDS before treatment were the influencing factors of GV(all P<0.05).Conclusion Short-term use of rhGH in children with GHD has a good effect,the height,IGF-1,and HtSDS before the treatment are the influencing factors of the therapeutic effect.
