摘要
脉诊与腹诊均属于切诊的范畴,为张仲景诊断疾病的重要方法,临床宜将脉诊与腹诊相结合,方能更全面地诊察疾病、判定证候,并有利于鉴别诊断,减少误诊的发生机率。脉诊与腹诊相结合,可进一步明确疾病的轻重(如大结胸证与小结胸证、大承气汤证与小承气汤证),鉴别诊断相类似的疾病(如痞证与结胸证、蓄水证与蓄血证),辨明虚实情况(如阳明腑实兼气血亏虚证),乃至鉴别某一个症状或体征(如腹急痛)。由是可见,张仲景在临床中极其重视运用脉诊与腹诊相结合的方法,以诊断疾病,辨明实质,值得进一步的学习与挖掘。
Both pulse diagnosis and abdominal diagnosis belong to the category of palpation,which is an important method for ZHANG Zhong-jing to diagnose diseases.It is appropriate to combine pulse diagnosis and abdominal diagnosis in clinical practice,in order to diagnose diseases and syndromes more comprehensively,and beneficial to facilitate differential diagnosis.Meanwhile,it can reduce the incidence of misdiagnosis.The combination of pulse diagnosis and abdominal diagnosis can further determine the severity of the diseases(such as big blocked-up chest syndrome versus small blocked-up chest syndrome,Dachengqi Decoction syndrome versus Xiaochengqi Decoction syndrome),identify and diagnose similar diseases(such as epigastric fullness syndrome and Blocked-up chest syndrome,water storage syndrome and blood storage syndrome),identify the deficiency syndrome and excess syndrome(such as acute Yangming Fu-viscera excess syndrome and qi and blood deficiency syndrome),and identify a specific symptom or sign(such as acute abdominal pain).Therefore,ZHANG Zhong-jing attaches great essential to the combination of pulse diagnosis and abdominal diagnosis in clinical practice to diagnose diseases and distinguish the essence,which is worthy of further study and exploration.
