摘要
目的:回顾性调查宁夏回族自治区妇幼保健院2018年儿科门诊处方超说明书用药现状,分析超说明书用药的影响因素及潜在原因,为促进儿科合理用药提供依据。方法:在医院信息系统(HIS)中随机抽取2018年儿科门诊处方,依据药品说明书内容,判定医嘱是否属于超说明书用药,并对抽样数据、超说明书用药类型、超说明书用药外延关系、质量控制等进行分析。结果:共抽取13995张处方,其中超说明书用药处方2369张(16.93%);用药记录25385条中超说明书用药记录3994条(15.73%);医师职称越高出现超说明书用药的情况越多,主任医师超说明书用药处方占44.07%;幼儿和学龄前儿童两个年龄段超说明书用药比例最高,分别为47.19%和29.39%;未提及儿童用药信息是最常见的超说明书用药类型,占44.21%;口服是最常见的超说明书给药途径;呼吸系统药物、抗感染药物、通便止泻药居超说明书药物类型的前三位。结论:宁夏回族自治区妇幼保健院儿科门诊超说明书用药情况比较严峻,是说明书修订滞后、临床合理用药监管不到位、规范用药指导不足等多因素作用的结果,急需各级管理部门合力规范和引导,以保障儿童用药安全。
Objective:To retrospective investigate the current status of off-label drug use in pediatric outpatient clinic of Ningxia Hui Autonomous Province Maternal and Child Health Hospital in 2018.The influencing factors and potential causes of off-label medication were analyzed to provide a basis for promoting rational drug use in pediatrics.Methods:The pediatrics outpatient prescriptions within a whole year of 2018 were randomly selected from the HIS system of Children's Hospital of Ningxia Hui Autonomous Region.According to the contents of the drug instructions,the doctor's orders were determined whether they were off-label drug use,and the sampling data,the type of off-label drug use,the extended relationship of over instruction drugs,and the quality control were analyzed.Results:Totally 13995 prescriptions were collected,2369 descriptions were off-label drug use,accounting for 16.93%.25385 medication records,including 3994 records were off-label use,accounting for 15.73%.The higher level the doctor's professional title,the more cases of off-label drug use,the chief physician's off-label drug use prescriptions accounted for 44.07%.Toddlers and preschoolers have the highest percentages of off-label medications at 47.18%and 29.39%,respectively.No mention of children's medication information is the most common off-label medication type,accounting for 44.21%,oral administration is the most common off-label medication route.Respiratory drugs,anti-infective drugs,laxatives and antidiarrheal drugs are the top three drug types of off-label drug use.Conclusion:The situation of off-label drug use in our hospital's pediatric outpatient clinic is relatively severe,which is the result of many factors,such as lagging in revision of the instructions,clinical rational drug supervision,and inadequate guidance for standardized medication.It is urgently needed for all levels of management departments to work together to standardize and guide,so as to ensure the safety of medication for children.
