摘要 目的探究天津市宝坻区孕妇外周血未成熟粒细胞(IG)的变化趋势并建立参考区间,探讨IG在不同年龄、孕期、胎次是否有显著差异。方法选取2019年1月1日至2019年6月30日该院产科就诊健康孕妇1235例为健康妊娠组,另选取同期同龄非妊娠健康妇女300例为对照组。按孕周将孕妇分为3组:≤12周前为孕早期组(46例),>12~27周为孕中期组(429例),>27周为孕晚期组(760例)。分别采集受试者空腹静脉血,采用Sysmex XN-2000全自动血细胞分析仪检测未成熟粒细胞绝对值(IG#)和未成熟粒细胞对白细胞的比例(IG%),比较健康妊娠组和对照组的IG差异,建立正常妊娠参考区间;同时比较不同年龄、孕期及胎次间IG的差异。结果健康妊娠组IG#参考区间为(0.000~0.257)×109/L,IG%参考区间为(0.00~2.40)%。健康妊娠组IG#和IG%高于对照组,差异有统计学意义(Z=-20.396,P<0.001;Z=-24.846,P<0.001)。对照组与不同孕期孕妇的IG差异:对照组<孕早期组<孕中期组和孕晚期组,差异有统计学意义(P<0.05)。不同年龄组间孕妇IG差异均无统计学意义(P>0.05)。第1胎与第2胎及以上孕妇间比较,IG#的差异无统计学意义(Z=-0.225,P=0.822),而IG%的差异有统计学意义(Z=-2.808,P=0.005)。结论孕妇外周血IG高于非妊娠健康女性,有必要建立孕妇IG的参考区间,孕早期IG低于孕中期和孕晚期,IG在不同年龄间无显著差异。 Objective To explore the change trend of immature granulocyte(IG)in peripheral blood of pregnant women in Baodi District of Tianjin,and to establish a reference interval to explore whether there are significant differences in IG among different ages,gestational periods and parity.Methods Totally 1235 healthy pregnant women from January 1,2019 to June 30,2019 were selected as healthy pregnancy group,and 300 non pregnant healthy women of the same age were selected as control group.According to gestational weeks,pregnant women were divided into three groups:early pregnancy group(≤12 weeks,46 cases),mid pregnancy group(>12-27 weeks,429 cases),late pregnancy group(>27 weeks,760 cases).Fasting venous blood was collected respectively,and the absolute value of immature granulocytes(IG#)and the ratio of immature granulocytes to leukocytes(IG%)were detected by Sysmex XN-2000 automatic hematology analyzer.The differences of IG between the two groups were compared,and the normal pregnancy reference interval was established.At the same time,the differences of IG among different ages,gestational periods and parity were compared.Results The reference interval of IG#in healthy pregnancy group was(0.000-0.257)×109/L,and the reference interval of IG%was(0.00-2.40)%.Compared with the control group,the difference of IG#and IG%in healthy pregnancy group was significantly increased(Z=-20.396,P<0.001;Z=-24.846,P<0.001).The difference of IG between the control group and different gestational periods:control group<early pregnancy group<mid pregnancy group and late pregnancy group(P<0.05).There was no significant difference in IG among different age groups(P>0.05).There was no significant difference in IG#between the first and second and above pregnant women(Z=-0.225,P=0.822),but there was significant difference in IG%(Z=-2.808,P=0.005).Conclusion It is necessary to establish the reference interval of IG in pregnant women.The IG in early pregnancy is lower than that in middle and late pregnancy.There is no significant difference between different ages.
