摘要
目的探讨基于授权理论的看图对话工具在初次胰岛素治疗糖尿病老年患者中的应用。方法选择2017年2月~2019年10月惠东县人民医院收治的186例糖尿病老年患者作为研究对象。按照随机信封法分研究组(93例)和对照组(93例)。对照组实施常规教育,研究组实施基于授权理论的看图对话工具教育。比较两组教育后生活习惯、行为变化、代谢指标、糖尿病知识认知程度评分。结果教育后,研究组坚持运动、遵医嘱用药、定期检查血糖、戒烟戒酒、合理饮食、定期复查的患者占比均高于对照组,差异有统计学意义(P<0.05);教育后,研究组血清总胆固醇、三酰甘油、舒张压、糖化血红蛋白、餐后2 h血糖、空腹血糖水平均低于对照组,差异有统计学意义(P<0.05);教育后,研究组胰岛素使用方法、糖尿病综合管理技能、并发症知识评分均高于对照组,差异有统计学意义(P<0.05)。结论对于糖尿病老年患者来说,在初次治疗中实施基于授权理论的看图对话工具教育,可以改善不良生活习惯及行为,改善代谢指标,提高患者对疾病知识的认识,具备显著临床价值。
Objective To explore the application of picture based dialogue tool based on empowerment theory in primary insulin treatment for elderly patients with diabetes.Methods A total of 186 elderly patients with diabetes who were admitted to Huidong People′s Hospital from February 2017 to October 2019 were selected as the research subjects.According to the random envelope method,they were divided into study group(93 cases)and control group(93 cases).The control group received routine education,while the study group received the picture dialogue tool education based on empowerment theory.The living habits,behavioral changes,metabolic indicators and diabetes knowledge cognition score after education were compared between the two groups.Results After education,the proportions of patients in the study group who adhered to exercise,took drugs according to the doctor′s advice,regularly checked their blood glucose,quit smoking and drinking,reasonable diet,and regularly reviewed were higher than those in the control group,the differences were statistically significant(P<0.05).After education,the levels of serum total cholesterol,triglyceride,diastolic blood pressure,glycosylated hemoglobin,postprandial 2 h blood glucose and fasting blood glucose in the study group were lower than those in the control group,the differences were statistically significant(P<0.05).After education,the scores of insulin use,diabetes comprehensive management skills and complications knowledge in the study group were higher than those in the control group,the differences were statistically significant(P<0.05).Conclusion For elderly patients with diabetes mellitus,in the first treatment,the implementation of the authorization theory based on the picture dialogue tool education,can improve bad habits and behaviors,improving metabolic indicators,raising patients′knowledge of disease,it has remarkable clinical value.
