摘要
目的探讨经下肢与上肢静脉PICC置管的效果及对新生儿疼痛程度、配合情况的影响。方法选取2017年5月至2019年5月收治的100例新生儿,以数字随机表法将其分为A组(n=50,经下肢静脉PICC置管)与B组(n=50,经上肢静脉PICC置管)。比较两组的置管效果。结果A组操作时间、出血量、留置时间均优于B组(P<0.05)。A组的一次穿刺成功率高于B组,导管异位率、送管困难率低于B组(P<0.05)。A组穿刺时、穿刺后5 min的疼痛程度明显轻于B组(P<0.05)。A组的配合度高于B组(P<0.05)。A组并发症总发生率低于B组(P<0.05)。结论对新生儿而言,经下肢静脉PICC置管较上肢静脉PICC置管效果更好,利于减轻患儿疼痛程度,减少置管后并发症的发生,提升患儿配合度及一次穿刺成功率,值得应用。
Objective To explore the effects of PICC catheter through lower extremity and upper extremity veins and its influences on pain degree and cooperation of newborns.Methods A total of 100 newborns admitted from May 2017 to May 2019 were selected and divided into group A(n=50,PICC catheter through lower extremity vein)and group B(n=50,PICC catheter through upper extremity vein)by digital random table method.The effect of catheterization was compared between the two groups.Results The operating time,blood loss volume and indwelling time of the group A were better than those of the group B(P<0.05).The success rate of one-time puncture of the group A was higher than that of the group B,and the rates of ectopic catheter and delivery difficulty of the group A were lower than those of the group B(P<0.05).The pain degree of the group A at puncture time and 5 min after puncture were significantly lighter than those of the group B(P<0.05).The coordination degree of the group A was higher than that of the group B(P<0.05).The total incidence of complications of the group A was lower than that of the group B(P<0.05).Conclusion For newborns,PICC catheter through lower extremity vein has better effect than PICC catheter through upper extremity vein,which is conducive to reducing the pain degree and the incidence of complications after catheterization,improving the cooperation degree and the success rate of one-time puncture.
