摘要
目的分析急性农药中毒患者的临床特征及预后影响因素。方法纳入我院收治的417例急性农药中毒患者,根据随访6个月内是否死亡分为存活组(370例)和死亡组(47例)。比较两组的临床特征、临床资料,分析影响急性农药中毒患者预后的危险因素。结果存活组的中枢神经系统症状、循环系统症状、呼吸系统症状发生率均显著低于死亡组(P<0.05);死亡组与存活组的中毒途径有显著差异(P<0.05)。院前时间、APACHEⅡ评分、农药剂量、院前急救率、农药类型、ChE、β-内啡肽、和肽素水平均为影响急性农药中毒患者预后的独立因素(P<0.05)。结论临床中对于急性农药中毒患者,应及时开展急救,并针对影响预后的独立因素及时开展对症治疗,以促进患者预后的改善。
Objective To analyze the clinical characteristics and factors affecting prognosis of patients with acute pesticide poisoning.Methods A total of 417 patients with acute pesticide poisoning admitted to our hospital were enrolled and divided into survival group(370 cases)and death group(47 cases)according to whether the patients died within 6 months of follow-up.The clinical characteristics and clinical data of the two groups were compared,and the risk factors affecting the prognosis of acute pesticide poisoning patients were analyzed.Results The incidences of central nervous system symptoms,circulatory system symptoms and respiratory system symptoms in the survival group were significantly lower than those in the death group(P<0.05);there was a statistically significant difference in poisoning route between the death group and the survival group(P<0.05).Pre-hospital time,APACHEⅡ score,pesticide dosage,pre-hospital emergency rate,pesticide type,ChE,β-endorphin and copeptin levels were all independent factors affecting the prognosis of acute pesticide poisoning patients(P<0.05).Conclusion For patients with acute pesticide poisoning,emergency treatment should be carried out in time,and symptomatic treatment should be carried out according to the independent factors affecting the prognosis,so as to improve the prognosis of patients.
