摘要
目的探讨性激素六项和性激素球蛋白对诊断男性梗阻和非梗阻无精子症的临床意义。方法选择2018年3月—2020年3月收治的98例行睾丸切开取精手术的无精子症患者作为研究对象,将非梗阻无精子症患者作为A组(50例),将梗阻无精子症患者作为B组(48例),同期选择50例正常生育男性作为对照组。对3组进行性激素六项和性激素球蛋白检测,比较3组性激素六项与性激素球蛋白水平。结果对照组睾酮、孕酮、雌二醇高于A组和B组,促卵泡刺激素、催乳素、性激素球蛋白水平低于A组与B组,黄体生成素高于B组,且低于A组,差异具有统计学意义(P<0.05);A组黄体生成素、促卵泡生成素、性激素球蛋白水平高于B组,差异具有统计学意义(P<0.05)。A组中黄体生成素水平、促卵泡生成素水平与性激素球蛋白水平具有相关性(P<0.05),在B组中黄体生成素水平与性激素球蛋白水平无显著相关性(P>0.05),促卵泡生成素水平与性激素球蛋白水平呈显著相关性(P<0.05)。结论黄体生成素、促卵泡刺激素、性激素球蛋白可作为诊断男性梗阻和非梗阻无精子症重要指标,可以反映无精子症严重程度,有效鉴别男性梗阻无精子症与非梗阻无精子症。
Objective To investigate the clinical significance of sextant and sex hormone globulin in the diagnosis of male obstructive azoospermia and non obstructive azoospermia.Methods From March 2018 to March 2020,98 patients with azoospermia undergoing orchiectomy and semen extraction were selected as research samples.Patients with non obstructive azoospermia were selected as group A(50 cases),patients with obstructive azoospermia as group B(48 cases),and 50 cases of normal fertile men were selected as control group.Six items of sex hormone and sex hormone globulin were detected in the three groups.Results The levels of testosterone,progesterone and estradiol in control group were higher than those in group A and group B,FSH,prolactin and sex hormone globulin were lower than those in group A and group B,luteinizing hormone was higher in group B and lower than that in group A,the difference was statistically significant(P<0.05);the levels of luteinizing hormone,follicle stimulating hormone and sex hormone globulin in group A were higher than those in group B(P<0.05).In group A,luteinizing hormone,follicle stimulating hormone and sex hormone globulin were correlated(P<0.05),while in group B,there was no significant correlation between luteinizing hormone level and sex hormone globulin(P>0.05),but there was a significant correlation between FSH level and sex hormone globulin level(P<0.05).Conclusion Luteinizing hormone(LH),follicle stimulating hormone(FSH)and sex hormone globulin can be used as important indicators in the diagnosis of male obstructive azoospermia and non obstructive azoospermia,which can reflect the severity of azoospermia,and effectively distinguish male obstructive azoospermia from non obstructive azoospermia.
