摘要
目的探讨老年男性血清性激素水平与冠心病(coronary artery disease,CAD)及其危险因素的关系。方法选取40例经冠状动脉造影术确诊为冠心病的老年男性和100例正常对照组。测定血清睾酮(T)、性激素结合球蛋白(SHBG)、雌二醇(E2)、硫酸脱氢表雄酮(DHEA-S)水平和血脂、空腹血糖(FBG)、空腹胰岛素水平(FINS),测量身高、体重、腰围、血压。结果CAD组血清T、SHBG、FT水平均显著者低于对照组。血清T、SHBG水平与HOMA-IR呈负相关(r=-0.211,-0.206);与体重指数(body mass index,BMI)呈负相关(r=-0.310,-0.212);与高密度脂蛋白胆固醇(High density lipoprotein-cholesterol,HDL-C)呈正相关(r=0.219,0.286);与低密度脂蛋白胆固醇(Low density lipoprotein-cholesterol,LDL-C)呈负相关(r=-0.226,-0.307);与脂蛋白a(Lp<a>)呈负相关(r=-0.198,-0.213)。logistic回归分析显示:T与CAD呈负相关。T每增加1SD,CAD的危险度下降41%(OR=0.59)。结论睾酮在冠心病的发展过程中起保护作用,老年男性内源性睾酮水平降低可增加男性冠心病发生危险性。
Objective To explore the relationship between sex hormones and coronary artery disease in aging men. Methods 40 cases with coronary artery disease according to coronary angiography, and 100 healthy male individuals were selected. Serum concentrations of testosterone(T), Sex hormone bingding globulin(SHBG), estradiol (E2), dehydroepiandrosterone sulfate (DHEA-S), lipids, fasting glucose, fasting insulin were measured. Body height, weight, waist circumference, blood pressure were assessed. Results Serum concentrations of T, SHBG and FT were lower in CAD group compared with control group. Serum T and SHBG levels were inversely related to HOMA-IR (r=-0.211, -0.206); inversely related to BMI (r=-0.310,-0.212); positively related to HDL-C (r=-0.219, 0.286); inversely related to LDL-C (r=-0.226, -0.307) and inversely related to Lp (a)(r=-0.198,-0.213). Logistic regression analyses showed an inverse relationship for T with CAD.Each SD increase in T was associated with a 41% reduced risk of having the metabolic syndrome (OR=0.59). Conclusions T plays a protective role in the development of CAD.Low endogenous T level appears to increase the risk of CAD in aging men.
