摘要
目的探究半夏白术天麻汤联合硝苯地平治疗老年高血压的临床疗效和对患者不良反应的影响。方法选取2018年10月至2019年12月收治的原发性高血压患者88例,随机分为对照组和研究组,每组44例。对照组患者使用硝苯地平控释片治疗,研究组在对照组患者用药的基础上加用半夏白术天麻汤治疗。比较2组患者的临床疗效和不良反应发生情况。比较2组患者治疗前后血压、内皮功能、肝肾功能变化情况。结果研究组患者治疗有效率为93.18%,显著高于对照组患者(75.00%)(P<0.05)。治疗后,2组患者的收缩压和舒张压均显著降低,研究组患者收缩压和舒张压均显著低于对照组患者(P<0.05)。治疗后,研究组的NO显著升高,且研究组患者NO水平显著高于对照组患者,2组患者的ET-1水平均显著降低,且研究组患者的ET-1水平显著低于对照组患者(P<0.05)。研究组患者不良反应发生率为18.18%,显著低于对照组患者(40.91%)(P<0.05)。治疗后,2组患者TBIL、Scr、BUN水平均显著上升,但研究组患者Scr、BUN水平显著低于对照组(P<0.05),而2组患者治疗后TBIL水平差异仍无统计学意义(P>0.05)。结论半夏白术天麻汤联合硝苯地平治疗老年高血压患者能够达到更为理想的临床疗效,降低患者血压,提高患者血管内皮功能状态,且联合治疗的不良反应发生率较低,对患者肝肾的损伤较低,治疗的安全性更高。
Objective To investigate the effects of Banxia Baizhu Tianma decoction combined with nifedipine in treatment of hypertension in elderly patients and its influences on adverse reactions of patients.Methods A total of 88 patients with essential hypertension who were admitted and treated in our hospital from October 2018 to December 2019 were enrolled in the study,who were divided into control group and research group,with 44 cases in each group.The patients in control group were treated by nifedipine sustained release tablets,however,the patients in research group,on the basis of control group,were treated by Banxia Baizhu Tianma Decoction.The clinical efficacy and adverse reactions were observed and compared between the two groups.Moreover the changes of blood pressure,endothelial function,liver and kidney function before and after treatment were observed and compared between the two groups.Results The effective rate in reseach group was significantly higher than that in control group(93.18%vs 75.00%,P<0.05).After treatment,the systolic pressure and diastolic pressure in both groups were significantly decreased,moreover,which in research group were significantly lower than those in control group(P<0.05).After treatment,the NO levels in research group were significantly increased,moreover,which in research group were significantly higher than those in control group.In addition the ET 1 levels in both groups were significantly decreased,which in research group were significantly lower than those in control group(P<0.05).In addition the incidence rate of adverse reactions in research group was significantly lower than that in control group(18.18%vs 40.91%,P<0.05).After treatment,the levels of TBIL,Scr and BUN in both groups were significantly increased,moreover,the levels of Scr and BUN in research group were significantly lower than those in control group(P<0.05).After treatment there were no significant differences in TBIL levels between the two groups(P>0.05).Conclusion Banxia Baizhu Tianma decoction combined with nifedipine in treatment of hypertension in elderly patients can obtain satisfactory theraeutic effects,which can decrease the blood pressure of patients and improve endothelial function status,and decrease the incidence of adverse reactions and relieve the damage on liver and kidney,with better safety.
