摘要
目的分析Ilizarov骨短缩-延长技术治疗无血管损伤的胫骨骨与软组织缺损的效果。方法回顾性分析2013年8月—2017年3月无锡市第九人民医院采用Ilizarov骨短缩-延长技术治疗的19例无血管损伤的胫骨骨与软组织缺损患者临床资料,男性11例,女性8例;年龄26~57岁,平均42.3岁。软组织缺损范围5cm×3cm~12cm×5cm,骨缺损长度5~11.5cm,平均7.14cm。统计创面愈合时间、骨愈合时间、患肢功能恢复情况和术后并发症情况。结果本组创面愈合时间为(20.9±6.8)d,骨缺损端骨靠拢时间为(25.8±16.7)d,延长段骨愈合时间为(8.8±1.1)个月,缺损端愈合时间为(8.9±1.4)个月。Paley标准评价骨折愈合疗效优良率94.7%,美国特种外科医院(HSS)膝关节评分优良率89.5%,踝关节Baird-Jackson评分优良率84.2%。Ilizarov手术主要并发症(骨对合端不愈合、邻近关节挛缩或僵硬等)发生率14.28%(4/19),次要并发症(钉道感染松动、软组织下陷等)发生率52.63%(10/19)。结论Ilizarov骨短缩-延长技术治疗无血管损伤的胫骨骨与软组织缺损有独特的优越性,手术操作简单、功能恢复良好。
Objective To investigate the clinical effect of bone shortening-lengthening by Ilizarov technique in the treatment of tibia bone and soft tissue defect without vascular injury.Methods A retrospective analysis was conducted in 19 patients with tibial defects of bone and soft tissue without vascular injury who were treated by bone shortening-lengthening with Ilizarov technique in Wuxi 9th People’s Hospital from Aug.2013 to Mar.2017.They were 11 males and 8 females,with an average age of 42.3 years(range,26 to 57 years).The soft tissue defect range was 5cm×3cm-12cm×5cm,and the bone defect length was 5-11.5cm,with an average of 7.14cm.The wound healing time,bone healing time,function recovery of lower extremity and complications were observed postoperatively.Results The healing time of wound was(20.9±6.8)days,the closure time of bone defect ends was(25.8±16.7)days,the healing time of lengthening site was(8.8±1.1)months,and the healing time of the defect site was(8.9±1.4)months.The Paley criteria evaluation system showed that the excellent and good rate of fracture healing was 94.7%,the excellent and good rate of the knee joint score of the American Hospital of Special Surgery(HSS)was 89.5%,and the excellent and good rate of the ankle joint Baird-Jackson score was 84.2%.The major complication rate of Ilizarov operation was 14.28%(4/19),and the minor complication rate was 52.63%(10/19).Conclusion In the treatment of tibial bone and soft tissue defects without vascular injury,bone shortening-lengthing by Ilizarov technique has advantages such as simple operation and good functional recovery.
